摘要
目的探讨腹腔镜胆囊切除手术中喉罩通气全身麻醉的临床效果。方法选取2016年1月—2017年9月腹腔镜胆囊切除手术患者90例以数字表法分组。气管插管全身麻醉组进行气管插管全身麻醉,喉罩通气全身麻醉组开展喉罩通气全身麻醉。比较两组麻醉优良率;气腹后、胆囊切除后和拔管15分钟后的血糖浓度;麻醉前后腹腔镜胆囊切除手术患者平均心率、平均动脉压;不良反应发生率。结果两组的麻醉优良率相似,P> 0.05。麻醉前两组平均心率、平均动脉压相近,P> 0.05;麻醉后喉罩通气全身麻醉组平均心率、平均动脉压高于气管插管全身麻醉组,P <0.05。喉罩通气全身麻醉组气腹后、胆囊切除后和拔管15分钟后的血糖浓度短于气管插管全身麻醉组,P <0.05,喉罩通气全身麻醉组不良反应发生率低于气管插管全身麻醉组,P<0.05。结论喉罩通气全身麻醉在腹腔镜胆囊切除手术麻醉中的应用效果确切,可减轻对腹腔镜胆囊切除手术患者血流动力学的影响,可维持生命体征稳定和减少不良反应发生。
Objective To investigate the clinical effect of laryngeal mask ventilation and general anesthesia in laparoscopic cholecystectomy. Methods From January 2016 to September 2017, 90 patients undergoing laparoscopic cholecystectomy were divided into two groups by digital table. The general anesthesia group received general anesthesia through tracheal intubation for patients undergoing laparoscopic cholecystectomy. The general anesthesia group received general anesthesia through laryngeal mask ventilation for patients undergoing laparoscopic cholecystectomy. The excellent rate of anesthesia, blood sugar concentration after pneumoperitoneum, cholecystectomy and 15 minutes after extubation, average heart rate and arterial pressure before and after anesthesia in patients undergoing laparoscopic cholecystectomy, and incidence of adverse reactions were compared between the two groups. Results The excellent and good rate of anesthesia in the two groups was similar, P > 0.05. The average heart rate and arterial pressure of the two groups before anesthesia were similar(P > 0.05);the average heart rate and arterial pressure of the laryngeal mask ventilation general anesthesia group after anesthesia were higher than those of the tracheal intubation general anesthesia group(P < 0.05). After pneumoperitoneum, cholecystectomy and extubation for 15 minutes, the blood sugar concentration in laryngeal mask ventilation general anesthesia group was shorter than that in tracheal intubation general anesthesia group(P < 0.05). The incidence of adverse reactions in laryngeal mask ventilation general anesthesia group was lower than that in tracheal intubation general anesthesia group(P < 0.05). Conclusion The application of laryngeal mask ventilation general anesthesia in laparoscopic cholecystectomy is effective, which can reduce the hemodynamic impact on patients undergoing laparoscopic cholecystectomy, maintain the stability of vital signs and reduce the occurrence of adverse reactions.
