维生素D与心脑血管疾病高危因素的孟德尔随机化研究 被引量:1

摘要 目的采用孟德尔随机化方法,分析血清维生素D含量与心脑血管疾病高危因素之间是否存在潜在的因果关联。方法在常州市武进区2万余自然人群队列研究中随机抽取样2878名,基于孟德尔随机化的方法,以全基因组关联研究确定的4个维生素D相关遗传变异构建加权遗传风险评分,采用线性回归模型估计维生素D与血压、空腹血糖、血脂等心脑血管高危因素的关系。结果在自然人群队列中,维生素D与心脑血管疾病高危因素(血压、空腹血糖和血脂)不存在统计学关联(收缩压:P=0.735;舒张压:P=0.407;空腹血糖:P=0.132;胆固醇:P=0.720;甘油三酯:P=0.865;高密度脂蛋白胆固醇:P=0.453),不论是单个遗传变异还是多位点构建的加权遗传评分(wGRS)均与上述高危因素无统计学关联(P值均>0.05)。结论孟德尔随机化研究结果提示维生素D与心脑血管高危因素无显著统计学关联,尚不支持补充维生素D作为预防心血管疾病干预措施的研究假设。 Objective To study whether vitamin D percentage(VD%)is causally associated with cardiovascular risk factors by Mendelian randomization analysis.Methods A total of 2878 patients randomly chosen from the 20000-people cohort from Wujin district of Changzhou city,the weighted genetic risk score(wGRS)was constructed based on 4 vitamin D-related genetic variations determined by genome-wide association study.The linear regression model was used to estimate the relationship between vitamin D and cardiovascular and cerebrovascular risk factors such as blood pressure,fasting blood glucose and blood lipid based on Mendelian randomization method.Results There was no significant correlation between vitamin D and high risk factors of cardiovascular and cerebrovascular diseases(blood pressure,fasting blood glucose and blood lipid)in the natural population cohort(systolic blood pressure:P=0.735;diastolic blood pressure:P=0.407;fasting blood glucose:P=0.132;cholesterol:P=0.720;triglyceride:P=0.865;HDL cholesterol:P=0.453).No matter it was a single genetic variation or a multi site weighted inheritance,there was no significant statistical correlation between wGRS and the above risk factors(all P>0.05).Conclusion The study has not found a significant association between VD% and cardiovascular risk factors by Mendelian randomization method,which does not support the hypothesis that vitamin D supplementation is an intervention to prevent cardiovascular disease.
