PCOS患者IVF-ET助孕结局的影响因素分析

摘要 目的:分析多囊卵巢综合征(PCOS)患者体外受精-胚胎移植(IVF-ET)助孕结局的影响因素。方法:选取2021年1月—2022年12月在赣州市妇幼保健院进行IVF-ET助孕的91例PCOS患者,随访6个月,统计患者助孕结局(临床妊娠率、未妊娠率),比较不同临床特征和不同助孕结局患者抗米勒管激素(AMH)、孕酮(P)、催乳素(PRL)、黄体生成素(LH)、睾酮(T)、雌二醇(E_(2))、卵泡刺激素(FSH)水平,采用logistic回归分析PCOS患者临床妊娠的影响因素。结果:91例PCOS患者中有68例临床妊娠、23例未妊娠;临床妊娠和未妊娠患者P、PRL、LH、T、年龄、PCOS病程水平比较,差异均无统计学意义(P>0.05);临床妊娠患者体重指数(BMI)低于未妊娠患者,血清AMH、E_(2)、FSH水平均高于未妊娠患者,差异均有统计学意义(P<0.05)。多因素logistic回归分析显示,AMH、E_(2)、FSH水平降低是影响PCOS患者临床妊娠的危险因素(P<0.05)。结论:AMH、E_(2)、FSH水平降低是影响PCOS患者临床妊娠的危险因素,助孕时可通过血清AMH、E_(2)、FSH水平预测临床妊娠。 Objective:To analyze the influencing factors of in vitro fertilization-embryo transfer(IVFET)assisted pregnancy outcomes in patients with polycystic ovary syndrome(PCOS).Method:A total of 91 PCOS patients who underwent IVF-ET assisted pregnancy in Ganzhou Maternal and Child Health Hospital from January 2021 to December 2022 were selected,followed up for 6 months,and their assisted pregnancy outcomes(clinical pregnancy rate and non-pregnancy rate)were analyzed.The levels of anti-miller tube hormone(AMH),progesterone(P),prolactin(PRL),luteinizing hormone(LH),testosterone(T),estradiol(E_(2))and follicle stimulating hormone(FSH)in patients with different clinical characteristics and different assisted pregnancy outcomes were compared,and the influencing factors of clinical pregnancy in PCOS patients were analyzed by logistic regression.Result:Among the 91 PCOS patients,68 were clinically pregnant and 23 were not pregnant.There were no significant differences in the levels of P,PRL,LH,T,age and PCOS course between clinically pregnant and non-pregnant patients(P>0.05).Body mass index(BMI)of clinically pregnant patients was lower than that of non-pregnant patients,and serum AMH,E_(2) and FSH levels were higher than those of non-pregnant patients,the differences were statistically significant(P<0.05).Multiple logistic regression analysis showed that the decreased levels of AMH,E_(2) and FSH were risk factors for clinical pregnancy in PCOS patients(P<0.05).Conclusion:Elevated levels of AMH,E_(2) and FSH are protective factors affecting clinical pregnancy in PCOS patients,and clinical pregnancy can be predicted by serum AMH,E_(2) and FSH levels during pregnancy.
