摘要
糖尿病视网膜病变是糖尿病最常见和严重的微血管并发症之一。早期病变以视网膜血管瘤、出血和渗出为主;晚期表现为新生血管形成、纤维组织增生和视网膜脱离。严格控制血糖、血压是控制其进展的基础,激光光凝和玻璃体视网膜手术为其基本治疗方法,尽管其疗效确切,但也存在局限性。陈志强教授认为本病以气阴两虚为本,目络湿瘀互结为标,其治疗当以"益气养阴,祛湿通络"为治则,活血通络贯穿始终。同时结合临床症状与眼底局部病变不同,辨证施治,每能获效。
Diabetic retinopathy is one of the most common and serious microvascular complications of diabetes mellitus.Early lesions were mainly retinal hemangioma,hemorrhage and exudation;Late stage showed neovascularization,fibrous tissue hyperplasia and retinal detachment.Strict control of blood sugar and blood pressure is the basis for controlling its progress.Laser photocoagulation and vitreoretinal surgery are the basic treatment methods.Although their curative effect is exact,there are limitations.Professor CHEN Zhi-qiang believes that the disease is based on qi and yin deficiency,and the combination of phlegm and blood stasis is manifestation.The treatment is based on the principle of’benefiting qi and nourishing yin,eliminating dampness for removing obstruction in collaterals’,and promoting blood circulation for removing obstruction in collaterals runs through.At the same time,according to the clinical symptoms and local lesions of the fundus,dialectical treatment,each can be effective.
