摘要
目的探讨靶控输注(TCI)丙泊酚时舒芬太尼抑制帝视内窥镜气管插管反应的半数有效浓度(EC50)。方法共纳入25例择期全麻手术患者,年龄20~60岁,ASAⅠ~Ⅱ级,BMI 18.5~25 kg/m^2,Mallampati气道分级Ⅰ~Ⅱ级。以效应室浓度靶控输注舒芬太尼,以血浆浓度为3.0μg/mL靶控输注丙泊酚,采用帝视内窥镜进行气管插管。采用序贯法设置舒芬太尼的效应室靶浓度:第一例患者舒芬太尼效应室靶浓度为0.36 ng/mL,相邻两例患者舒芬太尼效应室靶浓度对数值之差为0.05。舒芬太尼的EC50及95%可信区间(95%CI)用序贯法公式及Probit回归模型计算得出。结果舒芬太尼抑制帝视内窥镜气管插管反应的EC50用序贯法公式计算结果为0.29 ng/mL,95%可信区间是0.27~0.31 ng/mL。用Probit回归模型分析结果为0.29 ng/mL,95%可信区间是0.24~0.32 ng/mL。结论在靶控输注丙泊酚3.0μg/mL时,舒芬太尼抑制帝视内窥镜插管反应的EC50为0.29 ng/mL。
Objective To explore the Fifty Percent of Effective Concentration(EC50)of Sufentanil’s blunting responses to tracheal intubation performed by Disposcope endoscope during Target Controlled Infusion(TCI)of Propofol.Methods A total of twenty-five patients aged 20 to 60 who underwent selective surgery under general anesthesia were selected.Their health conditions ranked I to II according to the standard of ASA,BMIs ranged from 18.5 to 25 kg/m^2,and Mallampati airway scores indicated I to II.Sufentanil was delivered by TCI at effect-site concentration while Propofol was delivered by TCI at plasma concentration of 3.0μg/mL.Disposcope endoscope was employed to conduct endotracheal intubation.The target effect-site concentration of Sufentanil was determined by sequential method:the first patient was 0.36 ng/mL of Sufentanil and the difference value of logarithm of target effect-site concentration for the two adjacent patients was 0.05.The EC50 and95%confidence interval(95%CI)of Sufentanil were calculated by sequential method and Probit regression model.Results The EC50 of Sufentanil in blunting responses to tracheal intubation performed by Disposcope endoscope was 0.29 ng/mL(95%CI,0.27~0.31 ng/mL)by sequential method while that was 0.29 ng/mL(95%CI,0.24~0.32 ng/mL)by Probit regression model.Conclusion The EC50 of Sufentanil’s blunting responses to tracheal intubation by Disposcope endoscope is0.29 ng/mL during TCI of Propofol at concentration of 3.0μg/mL.
