锦州市接受抗病毒治疗的HIV/AIDS患者免疫重建及其影响因素分析

摘要 目的了解锦州市免疫缺陷病毒(Human immunodeficiency virus,HIV)感染者和艾滋病(AIDS)病人接受高效逆转录病毒疗法(Highly active antiretroviral therapy)后免疫重建的影响因素。方法收集中国疾病预防控制信息系统中2014年12月1日至2018年12月31日现住址在锦州市自愿接受HAART的HIV感染者和AIDS病人的资料。依据研究对象在不同治疗时间段CD4+T细胞计数情况和增长情况,采用重复测量资料的两水平生长模型,拟合T细胞计数变化情况,并分析其影响因素。结果共纳入87例研究对象,CD4+T细胞均值为(243.64±204.33)个/mm^3,治疗12个月和超过2年的患者分别上升到(495.18±262.93)和(591.84±238.40)个/mm^3。两水平模型拟合结果显示,入组年龄、性别、婚姻状况、基线CD4+T细胞计数、基线HIV病毒载量、感染途径、WHO临床分期均无统计学意义(P>0.05),但治疗时间的三次项系数(σ2μ1)差异有统计学意义(P<0.01)。结论锦州市艾滋病患者接受HAART治疗后的免疫重建效果明显,且不受其他因素的影响。 Objective To understand the influence factors of immune remodeling in patients with HIV infection and AIDS in Jinzhou city after receiving highly active antiretroviral therapy(HAART).Methods The study data of HIV infected persons and AIDS patients who received HAART voluntarily with current address of Jinzhou city during December 1,2014-December 31,2018 was collected from Chinese Disease Prevention and Control Information System.According to the CD4+T-cell count and growth situation in different treatment periods,the 2-level growth model with repeated measurements was applied to fit the change of CD4+T-cell count and analyze its influencing factors.Results A total of 87 cases were enrolled.The mean number of CD4+T cells was(243.64±204.33)/mm^3,which increased to(495.18±262.93)/mm^3 after 12 months treatment and(591.84±238.40)/mm^3 after over 2 years treatment.The results of the 2-level model fitting showed that age,gender,marital status,CD4+T cell count baseline,HIV viral load baseline,infection route and WHO clinical stage were not statistically significant(P>0.05),but the 3-degree coefficient of treatment time(σ2μ1)was statistically significant(P<0.01).Conclusion The immune remodeling effect of HIV/AIDS patients after receiving HAART treatment is obvious in Jinzhou city,and is not affected by other factors.
