摘要
[目的]回顾性分析溃疡性结肠炎(UC)患者中性粒细胞与淋巴细胞比值(NLR)、中性粒细胞与血小板压积比值(NPR)及D-二聚体水平与UC活动度的关系。[方法]选取2016年6月~2019年6月在中国医科大学附属盛京医院消化内科住院的205例UC患者,其中活动期117例,缓解期88例,分析患者NLR、NPR、D-二聚体的水平,并评估与UC严重程度之间的关系,评价对UC活动期诊断的特异性、敏感性及cut-off值。[结果]与缓解期相比,活动期患者NLR、NPR、D-二聚体水平显著高于缓解期,差异有统计学意义(P<0.05)。诊断UC活动期的ROC曲线下面积分别为0.815、0.729、0.898,敏感性、特异性及cut-off值分别为NLR(75.2%、81.6%、2.07),NPR(70.9%、72.0%、15.49),D-二聚体(76.9%、88.6%、126.5)。随着UC活动程度增高,NLR、NPR、D-二聚体水平与UC活动期轻、中、重度呈正相关关系。[结论]NLR、NPR、D-二聚体水平对UC活动期的诊断及疾病活动程度的评估有重要临床价值。
[Objective]To analyze the relationship between the activity of UC and the ratio of neutrophils to lymphocytes(NLR),the ratio of neutrophils to platelets(NPR)and the level of D-dimer.[Methods]From January 2016 to June 2019,205 UC patients were selected from the Department of Gastroenterology,Shengjing Hospital Affiliated to China Medical University,including 117 patients in active period and 88 patients in remission period.The levels of NLR,NPR and D-dimer in the two groups were analyzed,and the relationship between them and the severity of UC was evaluated.The specificity,sensitivity and cut-off value of the evaluation for the diagnosis of UC in active period were evaluated.[Results]Compared with the remission stage,the levels of NLR,NPR and D-dimer in the active stage were significantly higher than those in the remission stage(P<0.05).The area under the ROC curve for the diagnosis of UC in the active stage was 0.815,0.729 and 0.898,respectively.The sensitivity,specificity and cut-off values were NLR(75.2%,81.6%,2.07),NPR(70.9%,72.0%,15.49),and D-dimer Body(76.9%,88.6%,126.5).With the increase of UC activity,NLR,NPR,D-dimer were positively correlated with mild,moderate and severe activities.[Conclusion]NLR,NPR,D-dimer have important clinical value in the diagnosis of active stage of ulcerative colitis and the evaluation of disease activity.
