摘要
目的探究光动力疗法(PDT)联合手术治疗皮肤基底细胞癌(BCC)的疗效及安全性。方法将60例BCC患者,随机分为观察组(30例)和对照组(30例)。对照组采用外科手术进行治疗,观察组在对照组基础上加用PDT进行治疗。比较2组患者的不良反应、免疫指标、美容效果及疗效。结果观察组复发率及不良反应发生率显著低于对照组(3.33%vs 26.67%,6.67%vs 30.00%),P<0.05;观察组美容效果优良率显著高于对照组(90.00%vs 63.33%),P<0.05;2组患者的总有效率无统计学差异(96.67%vs 86.67%),P>0.05。治疗后,2组患者CD4^+、CD4^+/CD8^+水平均升高,且观察组CD4^+、CD4^+/CD8^+水平均显著高于对照组,2组患者CD8^+水平均降低,且观察组CD8^+水平显著低于对照组(P<0.05)。结论光动力疗法联合手术治疗BCC,复发率低,不良反应少,且美容效果好,可能与其能有效改善患者细胞免疫功能有关。
Objective To explore the efficacy and safety of photodynamic therapy(PDT)combined with surgery for patients with basal cell carcinoma(BCC)of skin.Methods 60 patients with BCC were randomly divided into the observation group(n=30)and the control group(n=30).The control group was treated with surgery,and the observation group was treated with PDT on the basis of the control group.The adverse reactions,immune indicators,cosmetic effects and curative effects of the 2 groups were compared.Results The recurrence rate and the adverse reactions rate of the observation group were significantly lower than those of the control group(3.33%vs.26.67%,6.67%vs.30.00%,P<0.05).The excellent and good rate of cosmetic effect in the observation group were significantly higher than that of the control group(90.00%vs.63.33%,P<0.05).There was no significant difference in the total effective rate between the 2 groups(96.67%vs.86.67%,P>0.05).After treatment,the levels of CD4^+and CD4^+/CD8^+were increased in both groups,and the levels of CD4^+and CD4^+/CD8^+in the observation group were significantly higher than those of the control group The CD8^+level in the 2 groups were lower,and the CD8^+level in the observation group was significantly lower than that of the control group(P<0.05).Conclusion Photodynamic therapy combined with surgery for BCC has a low recurrence rate,few adverse reactions and good cosmetic effect,which may be related to effective improvement of cellular immunity in patients.
