摘要
目的观察5-氨基酮戊酸光动力疗法(ALA-PDT)联合手术治疗皮肤基底细胞癌的中远期疗效。方法前瞻性选取自2010年1月至2012年1月收治的80例皮肤基底细胞癌患者,按随机数表法将其分对照组和观察组,每组各40例。对照组患者仅给予手术治疗,观察组患者采用ALA-PDT联合手术治疗。比较两组患者治疗后的临床疗效、伤口愈合情况、美容效果、不良反应率及复发率。结果观察组患者治疗后的总有效率为97.5%,高于对照组(90.0%),差异无统计学意义(P>0.05);观察组治疗后伤口一期愈合率为95.0%,美容效果优良率为92.5%;对照组伤口一期愈合率为75.0%,美容效果优良率为75.0%。观察组一期愈合率及美容优良率均较对照组高,差异具有统计学意义(P<0.05);观察组不良反应总发生率及复发率分别为2.5%、2.5%,均低于对照组(15.0%、20.0%),组间比较差异具有统计学意义(P<0.05)。结论 ALA-PDT联合手术治疗皮肤基底细胞癌具有一期愈合率高、美容效果好、安全性高、复发率低等优点,值得在临床上推广应用。
Objective To explore the long-term efficacy of 5-aminolevulinic acid photodynamic therapy（ ALA-PDT） in the treatment of skin basal cell carcinoma. Methods A total of 80 patients with basal cell carcinoma were selected from January 2010 to January 2012,and the patients were divided into control group and observation group according to the random number table,40 cases in each group. The patients in the control group were treated with surgery alone. The patients in the observation group were treated with ALA-PDT. The clinical efficacy,wound healing,cosmetic effect,adverse reaction rate and recurrence rate were compared between the two groups. Results The total effective rate was97. 5% in the observation group,which was higher than that in the control group（ 90. 0%）. There was no significant difference between the two groups（ P 〈 0. 05）. The healing rate of the wound was 95. 0% The excellent and good rate was 92. 5%. The healing rate of wound healing was75. 0% in the control group and 75. 0% in the cosmetic group. The healing rate and the cosmetic excellent and good rate of the observation group were higher than those of the control group,the difference was statistically significant（ P 〈 0. 05）. The total incidence and recurrence rate of adverse events in the observation group were 2. 5% and 2. 5%,respectively,which were lower than those in the control group（ 15. 0% and 20. 0%）,and the difference was statistically significant（ P 〈 0. 05）. Conclusion ALA-PDT combined with surgical treatment of skin basal cell carcinoma has the advantages of high healing rate,good cosmetic effect,high safety and low recurrence rate,which is worthy of clinical application.
