摘要
目的:探讨基于动机性访谈的护理干预在慢性心力衰竭(CHF)患者中的应用及对自我效能、自护行为和生存质量的影响。方法:选取2018年3月-2019年6月本院心内科收治的152例CHF患者,根据患者意愿选择的护理干预方式分为干预组(n=72)与对照组(n=80)。对照组进行常规住院护理和出院指导,干预组在对照组基础上实施基于动机性访谈的护理干预。比较两组干预前后一般自我效能感量表(GSES)、心力衰竭自我护理指数量表(SCHFI)及明尼苏达心力衰竭生活质量量表(MLHFQ)评分。结果:干预后,干预组自我效能评分明显高于干预前与对照组(P<0.05)。干预后,干预组自护行为各维度评分及总分均高于干预前与对照组(P<0.05)。干预后,干预组生活质量各评分及总分均低于干预前与对照组(P<0.05)。结论:基于动机性访谈的护理干预能有效改善CHF患者的自我效能、自护行为和生存质量,可供临床借鉴。
Objective:To explore application of nursing intervention based on motivational interviewing in patients with chronic heart failure (CHF) and its influences on self-efficacy,self-care behavior and quality of life.Method:A total of 152 CHF patients admitted to the cardiology department of our hospital from March 2018 to June 2019 were selected.According to the nursing intervention method selected by the patients,they were divided into intervention group (n=72) and control group (n=80).The control group was given routine in-patient nursing and discharge guidance,and the intervention group was given nursing intervention based on motivational interviewing on the basis of the control group.The scores of general self-efficacy scale (GSES),self-care of heart failure index (SCHFI) and Minnesota living heart failure questionnaire (MLHFQ) were compared before and after the intervention between the two groups.Result:After intervention,the self-efficacy score of the intervention group was significantly higher than before the intervention and that of the control group (P<0.05).After the intervention,all dimension and total scores of self-care behavior in the intervention group were higher than before the intervention and those in the control group (P<0.05).After the intervention,all dimension and total scores of life quality in the intervention group were lower than before the intervention and those in the control group (P<0.05).Conclusion:Nursing intervention based on motivational interviewing can effectively improve self-efficacy,self-care behaviors and quality of life of CHF patients,which can be applied for clinical reference.
