摘要
目的探讨甲钴胺联合木丹颗粒治疗2型糖尿病患者周围神经病变(DPN)的临床疗效及安全性。方法选择2017年11月-2018年2月社区门诊收治的60例2型糖尿病合并DPN的患者,根据随机数字表分为观察组30例与对照组30例,对照组给予甲钴胺注射液,观察组给予甲钴胺注射液+木丹颗粒。比较12周治疗后患者DPN程度评分、空腹血糖(fasting plasma glucose, FPG)、餐后2小时血糖(2 hour Postprandial Blood Glucose, 2h PBG)、糖化血红蛋白(glycated hemoglobin, HbA1c)、谷丙转氨酶、肌酐及不良反应。结果两组患者治疗前年龄、性别比、糖尿病病程、糖化血红蛋白、DPN程度评分无显著差异(P>0.05)。两组治疗后糖化血红蛋白、餐后2小时血糖、DPN程度评分与治疗前相比有明显改善(P<0.001)。观察组与对照组相比治疗后DPN程度评分改善明显(P<0.05)。观察组和对照组患者均出现不良反应,但无统计学差异(P>0.05)。结论木丹颗粒是一种良好的协助甲钴胺治疗DPN的理想药物,且安全性良好,适合临床使用。
Objective To explore the clinical efficacy and safety of mecobalamin combined with Mudan granules in the treatment of diabetic peripheral neuropathy(DPN). Methods From November 2017 to February 2018, 60 patients with type 2 diabetes and DPN were admitted to the community clinic. They were divided into observation group and control group according to random number table. The control group was given methylcobalamin injection and the observation group was given methylcobalamin injection + Mudan granules. Comparison of DPN score, fasting plasma glucose(FPG), 2 hour postprandial blood glucose(2 h PBG), glycated hemoglobin(HbA1 c), alanine aminotransferase, creatinine and adverse reactions after 12 weeks of treatment. Results There was no significant difference in age, sex ratio, duration of diabetes, glycated hemoglobin, DPN degree scores before treatment between the two groups(P > 0.05). After treatment, glycated hemoglobin, 2 hour Postprandial Blood Glucose, and DPN degree scores were significantly improved compared with before treatment(P <0.001). Compared with the control group, the observation group improved the DPN degree score significantly(P < 0.05). Adverse reactions occurred in the observation group and the control group, but there was no statistical difference(P > 0.05). Conclusion Mudan granule is an ideal drug to assist mecobalamin in the treatment of DPN, and it has good safety and is suitable for clinical applications.
