摘要
目的通过对丹参酮ⅡA磺酸钠注射液说明书以外的配伍禁忌进行全方位的汇总和分析,提出应对措施,为临床安全用药提供体外配伍方面的建议。方法利用中国知网等数据库,进行全方位的检索,文献的筛选采用“同类取最新,独特全采纳”的原则,对说明书以外的配伍禁忌报道进行汇总,并分析发生配伍禁忌的可能原因。结果共计检索到20篇有效文献,涉及20余种。肉眼所能观察的如出现悬浮物、沉淀等配伍禁忌现象,其发生的主要原因可能和药液pH发生变化有直接关联。同时,对于肉眼观察不到的配伍禁忌也应当引起重视。建议临床医师在开具丹参酮ⅡA磺酸钠注射液时在该药前增加“冲管”的标识。护理人员在不增加患者生理负荷的前提下,间隔液量在30~40 mL,冲管时长在12~14 min为宜。结论药师通过对丹参酮ⅡA磺酸钠注射液说明书以外的配伍禁忌的汇总分析,提出合理建议,能够为临床医护人员安全使用该药提供实践依据,药师可以从不同的医疗实践角度发挥专业价值。
Objective To summarize the off-label incompatibility of sodium tanshinoneⅡA sulfonate injection,and put forward corresponding measures to provide suggestion for the safe use of sodium tanshinoneⅡA sulfonate injection.Methods The database of CNKI was completely retrieved.The principle of“selecting the newest from the same kind and adopting all the unique”was practiced in the literature selection.The offlabel incompatibility was summarized,and the possible reasons for incompatibility were analyzed briefly.Results Totally 20 effective literatures were found involving more than 20 varieties.Occurrence of suspension,precipitation and other in compatibilities observed by naked eyes might be directly related to changes in the pH of the drug solution.Meanwhile,the incompatibility that failed to be observed by naked eyes should also be paid attention to.We suggest that clinicians add a“flushing tube”label before prescribing sodium tanshinoneⅡA sulfonate injection,and the volume of interval fluid should be 30-40 mL and the duration time of flushing tube should be 12-14 minutes without increasing the physiological load of patients.Conclusion According to the summary and incompatibility analysis of sodium tanshinoneⅡA sulfonate injection,pharmacists can help doctors and nurses to use it safely.Pharmacists play a key role by using their professional knowledge from different medical practice perspectives.
