摘要
目的通过分析浙东北地区新生儿感染性结膜炎的流行病学特征、病原谱结构和药敏谱,为临床诊治新生儿感染性结膜炎提供依据。方法回顾2016年1月-2018年12月在慈溪市妇幼保健院住院的186例由产道感染和产后院内感染引起的新生儿结膜炎病例,就其流行情况、菌谱结构及抗生素敏感耐药谱加以探讨。结果186例新生儿结膜炎有57株由院内感染引起,占29%;139株由产道感染引起,占71%。共培养出致病菌196株,主要包括革兰阳性球菌74株,占37.76%,其次为革兰阴性杆菌(24.49%)、革兰阴性球菌(20.41%)、其他17.34%。三代头孢、喹诺酮类、氨基糖苷类对淋球菌、大肠埃希菌、葡萄球菌普遍较敏感;环丙沙星、万古霉素对肠球菌敏感。结论产道感染是新生儿感染性结膜炎的主要来源,其次为产后院内感染。葡萄球菌、大肠埃希菌、淋球菌是主要的致病菌。药敏结果提示致病菌对眼部常用药耐药率高低不一。
Objective To provide basis for clinical diagnosis and treatment of neonatal infectious conjunctivitis by analyzing the epidemiological characteristics,pathogen spectrum and drug sensitivity spectrum of neonatal infectious conjunctivitis in northeastern Zhejiang.Methods A retrospective study of 186 cases of neonatal conjunctivitis caused by birth canal infection and postpartum nosocomial infection in the Cixi Maternal and Child Health Hospital from January 2016 to December 2018 was conducted for the exploration of prevalence,spectrum structure and antibiotic resistance,spectrum.Results Of the 186 neonates with conjunctivitis,57 were caused by nosocomial infections,accounting for 29%;139 were caused by infections in the birth canal,accounting for 71%.A total of 196 pathogenic bacteria were cultivated,including 74 Gram-positive cocci,accounting for 37.76%,followed by Gram-negative bacilli(24.49%)and Gram-negative cocci(20.41%).Third-generation cephalosporins,quinolones,and aminoglycosides are generally sensitive to Neisseria gonorrhoeae,Escherichia coli,and Staphylococcus;ciprofloxacin and vancomycin are sensitive to Enterococcus.Conclusion The birth canal infection is the main source of neonatal infectious conjunctivitis,followed by postpartum nosocomial infection.Staphylococcus,Escherichia coli,and Neisseria gonorrhoeae are the main pathogens.The drug susceptibility results suggest that the resistance rate of pathogenic bacteria to common ocular drugs varies.
