维生素D水平与小儿肺炎炎症指标相关性分析 被引量:10

摘要 目的探究维生素D水平与小儿肺炎炎症指标的相关性。方法选取2015年5月至2018年5月东莞市常平医院收治的小儿肺炎168例为肺炎组,另选择同期来院进行健康体检正常小儿69例为对照组。比较两组的25-羟维生素D3(25-OH-D3)、C反应蛋白(CRP)、降钙素原(PCT)及白细胞(WBC)的水平。采用随机数字表法将肺炎组分成了肺炎A组与肺炎B组,肺炎A组给予阿奇霉素治疗,肺炎B组在肺炎A组的基础上加用维生素D治疗,比较两组治疗前后25-OH-D3、CRP、PCT及WBC的水平。分析肺炎患儿血清25-OH-D3与CRP、PCT及WBC之间的相关性,分析经过维生素D治疗后血清25-OH-D3与CRP、PCT及WBC之间的相关性。结果肺炎组与对照组治疗前相比血清25-OH-D3水平较低,CRP、PCT及WBC水平较高,差异均具有统计学意义(t=4.77~6.98,均P <0.05)。肺炎A组与肺炎B组治疗后分别与治疗前相比血清25-OH-D3水平升高,CRP、PCT及WBC水平降低;肺炎B组与肺炎A组治疗后相比血清25-OH-D3水平升高,CRP、PCT及WBC水平降低,差异均具有统计学意义(t=3.84~8.22,均P <0.05)。采用多元回归分析可见,血清25-OH-D3水平与CRP、PCT及WBC之间具有显著的相关性(回归系数=-2.97^-0.85,均P <0.05)。结论肺炎患儿维生素D水平与CRP、PCT及WBC等炎症指标水平失调相关,通过给予补充维生素D治疗,能够促进改善肺炎患儿体内25-OH-D3、CRP、PCT及WBC的水平。 Objective To explore the correlation between vitamin D level and inflammatory markers of pneumonia in children. Methods 168 cases of infantile pneumonia treated in our hospital during the period from May 2015 to May 2018 as pneumonia group;at the same time, 69 healthy babies were taken as control group. The levels of 25-hydroxy vitamin D3(25-OH-D3), c-reactive protein(C-reactive protein, CRP), calcitonin(procalcitonin, PCT) and white blood cells were compared between the two groups. The cases of pneumonia were divided into group A and group B according to the random number table. Group A was treated with azithromycin, and group B was additionally treated with vitamin D on the basis of group A treatment. Then, levels of 25-OH-D3, CRP, PCT and WBC were compared between the two groups before and after the treatment. The correlations between serum 25-OH-D3 and CRP, PCT and WBC in children with pneumonia were analyzed, and the correlations between serum 25-OH-D3 and CRP, PCT and WBC after the treatment with vitamin D were analyzed. Results Compared with the control group, before the treatment, the serum level of 25-OH-D3 in the pneumonia group was lower, and the levels of CRP, PCT and WBC were higher, with statistically significant differences(t = 4.77 ~ 6.98, P<0.05). In pneumonia group A and pneumonia group B, serum levels of 25-OH-D3 were increased and the levels of CRP, PCT and WBC were all decreased after the treatment, and the difference was statistically significant(t = 3.84 ~ 8.22, P<0.05). Multiple regression analysis showed that serum 25-OH-D3 level was significantly correlated with levels of CRP, PCT and WBC(regression coefficient =-2.97 ^-0.85, P<0.05 for all). Conclusion Vitamin D levels in children with pneumonia are associated with abnormal levels of inflammatory markers such as CRP, PCT, and WBC. Treatment with vitamin D supplementation can improve the levels of 25-OH-D3, CRP, PCT, and WBC in children with pneumonia.
