摘要
48例晚期非小细胞肺癌病人接受了异环磷酰胺加表阿霉素加顺铂(IEP)联合化疗方案。其中40例能评价疗效,包括Ⅳ期17例、Ⅲb期16例、Ⅲa期5例及Ⅱ期2例。部分缓解18例(45％),微小缓解(MR)或无变化(NC)9例(22.5％),无效(NR)13例。有效病例的生存期为5^+—19^+个月(中位时间12.4月)。MR和NC及NR病人的中位生存期为8.8月。所有病人在化疗后都有轻度到中度的恶心、呕吐。30％病例发生Ⅰ级和Ⅱ级白细胞减少,3例发生Ⅲ级白细胞减少,3例发生Ⅱ级和Ⅲ级血小板减少,25％病例有轻度(Ⅱ级)贫血。总之,IEP方案耐受性好,对非小细胞肺癌化疗有效。
Fourty-eight patients with advanced non-small cell lung carcinoma w-ere treated with IEP regimen. Ifosfamide 1.5gm/ m 2 / day was given by IV infusion over 4 hours together with Ⅳ bolus of mesna 400mg at time 0,4,8, hours post Ifosfamide on day 1 and 2. Epirubicin 60mg/ m 2 was given by slow IV injection on day 1. Cisplatin 60 mg/ m 2 was given by Ⅳ infusion over 4 hours on day 3. The treatment will be repeated every 3-4 weeks. Date is complete on 40 evaluable patients. There were 19 males and 21 males and 21 females with a median age of 58 years ( range 27-75). Karnofsky performance status was 70%( rang 50-80%). There were 27 adenocarcinomas, 11 squamous cell carcinomas, 2 anaplastic large cells. Seventeen cases were stage Ⅳ, sixteen cases were were stage Ⅲb, five cases were stage ma, two cases were stage Ⅱ. Partial response(PR) was observed in 18 cases (45%) . Minor response (MR)/ No change (NC) was achieved in 9 cases (22.5%) Thirteen cases were no response (NR). Survival time of the responders range from 5+ to 19+ months(Median=12.4 months). Median survival time of the MR/NC and NR was 8.8 months. Mild to moderate nausea and vomiting were seen in all casses. Grabe 1 and 2 leukopenia were seen in 30% of cases. Only three cases had grade 2 leuk-openia. Grade 2 and 3 thrombocytopenia were seen in 3 cases. Mild anemia (grade 2) w-ere seen in 25% of cases. In summary IEP regimen appears to be a well tolerated and active regime for non-small cell lung cancer.
