产ESBL及非产ESBL大肠埃希菌整合子分布差异与耐药性分析 被引量:6

Analysis of integron distribution and drug resistance of clinical isolates of ESBL-producing and non-ESBL-producing Escherichia coli

摘要 目的通过研究Ⅰ、Ⅱ、Ⅲ类整合子在临床分离的产超广谱β-内酰胺酶(ESBLs)与非产ESBLs大肠埃希菌中的分布情况,分析其与细菌耐药性的关系。方法收集并保存临床送检标本分离的产ESBLs大肠埃希菌49株,非产ESBLs大肠埃希菌40株;鉴定及药敏试验采用BDphoenix100全自动细菌鉴定仪;整合子及可变区测定采用聚合酶链反应(PCR)扩增和琼脂糖凝胶电泳,并对阳性菌株可变区进行测序分析并进行比对。结果89株大肠埃希菌中检测出Ⅰ类整合子阳性菌株共36株;其中,产ESBLs菌株的检出率为49.0%(24/49);非产ESBL大肠埃希菌的检出率为30%(12/40);未检出Ⅱ类及Ⅲ类整合子。有15株成功扩增出可变区。检出dfrA17、aadA5、aadA22、aadA2、dfrA12共5种耐药基因盒,与耐药表型有一定的相关性。结论Ⅰ类整合子在ESBLs和非产ESBLs的大肠埃希菌中的分布存在较为明显的差异,显示其与细菌的耐药性关系紧密,应给予足够重视。 Objective To study the distribution of class Ⅰ,Ⅱ and Ⅲ integrons in extended spectrum beta-lactamases (ESBLs)-producing and non-ESBLs-producing isolates of Escherichia coli and its relationship with bacterial resistance. Methods 49 strains of ESBLs-producing Escherichia coli and 40 strains of non-ESBLs-producing Escherichia coli isolated from clinical samples were collected and preserved.Bacterial identification and susceptibility test were conducted by BD phoenix100 automatic bacterial identification instrument.Polymerase chain reaction (PCR) and agarose gel electrophoresis were used to detect the integrase gene and variable region,and the variable regions of positive strains were sequenced and compared. Results A total of 36 class Ⅰ integron positive strains were detected in 89 strains of Escherichia coli.The detection rate of ESBLs-producing strains was 49.0%(24/49),and the detection rate of non-ESBLs-producing Escherichia coli was 30%(12/40).Class Ⅱ and class Ⅲ integrons were not detected.Variable regions in 15 strains were successfully amplified and analyzed.Five kinds of drug resistance gene cassettes,namely dfrA17,aadA5,aadA22,aadA2 and dfrA12,were detected,which were related to the resistant phenotype. Conclusion There is a significant difference in the distribution of class Ⅰ integrons between ESBLs-producing and non-ESBLs-producing Escherichia coli,indicating that it is closely related to bacterial resistance and should be given sufficient attention.
