TAP检测对乳腺癌患者预后判断的意义 被引量:15

摘要 目的探讨肿瘤异常糖链糖蛋白(TAP)作为乳腺癌预后判断指标的可能性。方法选取临床确诊并采用手术切除联合化疗治疗的乳腺癌患者52例,于治疗结束后复查时采集其静脉血并涂片,加入含有凝集素的检测试剂,血中含有的异常糖链糖蛋白凝集形成类晶体凝聚物,以最大凝集物面积为衡量指标,分析不同临床病理特征患者TAP表达的差异,比较治疗后未出现转移的乳腺癌患者组和发生转移的乳腺癌患者组TAP凝聚物面积的差异,评估TAP表达情况对乳腺癌患者预后判断的意义。结果与年龄<60岁乳腺癌患者相比,年龄≥60岁患者治疗后的TAP阳性表达率高,差异有统计学意义(χ~2=5. 127,P <0. 05); PR阴性乳腺癌患者较PR阳性患者的TAP阳性表达率高,差异有统计学意义(χ~2=4. 282,P <0. 05);与治疗后未发生转移的乳腺癌患者组相比,治疗后出现转移的乳腺癌患者组的TAP阳性表达率高,差异有统计学意义(χ~2=18. 326,P <0. 05);且治疗后出现转移的乳腺癌患者的TAP凝聚物面积[145. 9 (128,154. 7)μm^2]高于未发生转移的乳腺癌患者组[97. 6(79. 1,116. 8)μm^2],差异有统计学意义(Mann-Whitney U=52,P <0. 05)。结论 TAP检测可以用作乳腺癌患者预后判断的指标。 Objective To investigate the possibility of tumor abnormal glycan glycoprotein(TAP)as a prognostic marker for breast cancer.Methods Fifty-two breast cancer patients,who had undergone surgery and chemotherapy,were recruited when they rechecked in hospital after therapy.The venous blood was collected and prepared as smears.The detection reagent containing agglutinin was added onto the smears.The abnormal glycan glycoprotein in blood was agglutinated to form crystalloid aggregates,and the maximum agglutination area was used as an index of the content of TAP.Using this method,TAP content was compared among the patients with different clinicopathological characteristics,and the area of TAP aggregates was compared between the breast cancer patients without metastasis and those with metastasis after therapy for evaluating the value of TAP detection in the prognosis of breast cancer patients.Results Compared with breast cancer patients aged less than 60 years,the positive expression rate of TAP in breast cancer patients aged more than 60 years after treatment was increased(χ^2=5.127,P<0.05).The positive expression rate of TAP in PR-negative breast cancer patients was higher than that in PR-positive patients(χ^2=4.282,P<0.05).The positive expression rate of TAP in breast cancer patients with metastasis was higher than that in breast cancer patients without metastasis after therapy(χ^2=18.326,P<0.05).The area of TAP aggregates in breast cancer patients with metastasis was higher than that in breast cancer patients without metastasis after therapy[145.9(128,154.7)μm^2 vs 97.6(79.1,116.8)μm^2,Mann-Whitney U=52,P<0.05].Conclusion TAP detection can be used as a prognostic indicator for breast cancer patients.
