摘要
目的探讨D-二聚体及超声检查在评价下肢深静脉血栓(DVT)稳定性中的应用效果。方法选择DVT患者74例,根据回收的下腔静脉滤器内有无脱落血栓分为DVT不稳定组38例和DVT稳定组36例。比较两组患者的D-二聚体水平。采用受试者工作特征曲线分析D-二聚体判断DVT稳定性的效果。比较两组DVT超声特点,以及各组滤器置入前及取出术前的超声特点。结果 DVT不稳定组患者的D-二聚体水平高于DVT稳定组(P <0. 05)。D-二聚体判断DVT稳定性的曲线下面积为0. 815(P <0. 05)。两组的DVT超声特点差异无统计学意义(P> 0. 05)。在两组患者中,滤器取出术前的管腔血流充盈情况均优于置入术前(均P <0. 05)。结论 D-二聚体对DVT稳定性具有较好的诊断价值,但超声检查是否可应用于DVT稳定性的监测尚不确定。
Objective To explore the efficacy of D-dimer and ultrasonography applied to evaluating the stability of lower extremity deep venous thrombosis( DVT). Methods Seventy-four patients with DVT were divided into unstable DVT group( n = 38) and stable DVT group( n = 36) according to whether there was shedding thrombus in the recycled inferior vena cava filter. The D-dimer levels of the two groups were compared. Receiver operating characteristic curve was used to analyze the efficacy of D-dimer for determining the stability of DVT. The ultrasonic features of DVT of the two groups were compared,and the ultrasonic features of DVT before filter placement were compared with those before filter extraction in each group. Results The D-dimer level in the unstable DVT group was higher than that in the stable DVT group( P < 0. 05). The area under the curve of D-dimer for determining the stability of DVT was 0. 815( P < 0. 05). There was no statistically significant difference in the ultrasonic features of DVT between the two groups( P > 0. 05). Among the patients in both groups,the conditions of blood-filled veins before filter extraction were superior to those after filter placement( all P < 0. 05). Conclusion D-dimer has better value for determining the stability of DVT,but the feasibility of ultrasonography applied to monitoring the stability of DVT is still undetermined.
