摘要
目的针对新建三级医院的住院患者晨间血标本检验前流程,运用失效模式与效应分析(FMEA)进行改善,减少标本检验前高风险因素,降低标本拒收率、送检逾时率,提高医疗服务品质。方法运用FMEA血标本检验前流程风险评分表评估流程运行情况,探索流程失效的高风险因素,分析失效原因,采取改善措施。结果经FMEA改善后血标本拒收率由0.23%下降到0.11%,护理人员检验标本采集作业正确率由66.50%上升至92.35%,患者晨间血标本送检逾时率明显改善。结论 FMEA可明显改善新建三级医院住院患者晨间血标本检验前流程。
Objective To improve the pre-analysis procedures of morning blood sample from patients in newly-established tertiary hospitals.Failure mode and effects analysis(FMEA)were applied to decrease high-risk factors before sample analysis,minimize the rejection rate and overdue rate of sample,and enhance medical service quality.Methods The FMEA mark sheet on risks in pre-analysis procedures of blood sample was used to assess the operations,identify risk factors of process failure,analyze the reasons for process failure,and adopt measures of improvement.Results After the improvement of failure mode and effects analysis,the rejection rate of sample decreased from 0.23%to 0.11%,the accuracy of sample collection by nurses increased from 66.50%to 92.35%,and the overdue rate of morning blood sample was significantly improved.Conclusion FMEA can obviously improve the pre-analysis procedures of morning blood sample from patients in newly-established tertiary hospitals.
