摘要 目的探讨应用失效模式与效应分析方法改进手术器械处理流程的效果。方法根据卫生部新颁布的《清洗消毒及灭菌技术操作规范》设计手术器械处理流程,使用失效模式与效应分析方法对器械处理流程中可能出现的失效模式进行评估、分析,制定改进措施,并加以评价和改善。结果手术器械处理流程存在冲洗不彻底(RPN=512)、清洗流程错误(RPN=294)、未进行初步消毒(RPN=140)、精密仪器损耗(RPN=75)、器械生锈(RPN=63)、器械处理不及时(RPN=60)等高风险失效模式,经采用针对性的改进措施后,RPN值分别下降至125、0、0、12、12、8。结论应用失效模式与效应分析方法于手术器械处理流程改进的效果良好,使规范得以顺利实施。 Objective To discuss the application of failure mode and effects analysis in the process of instruments disposal in operation room.Methods The process of instrument disposal was designed with the guidance of newly published Standard for Operating Procedure of Cleaning,Disinfection and Sterilization by Ministry of Health.Possible failure mode in the process of instrument disposal was evaluated,analyzed,corresponding measures were taken then.Results There were some high-risk failure modes such as incomplete cleaning(RPN=512),wrong cleaning process(RPN=294),without initial disinfection(RPN=140),instrument deterioration(RPN=75),instrument rusting(RPN=63),without timely disposal(RPN=60) in operation room.RPN decreased to 125,0,0,12,12,and 8 respectively with related measures.Conclusion It is effective to apply failure mode and effect analysis in the process of instrument disposal.
