儿童1型糖尿病治疗中不同胰岛素注射方案效果探究 被引量:9

摘要 目的研究儿童1型糖尿病治疗中不同胰岛素注射方案效果。方法将于2010年2月—2017年2月就诊儿童1型糖尿病患儿92例根据治疗方式分组,每组46例,皮下注射组采用多次皮下胰岛素注射治疗(3~4次)的方案;泵注组则采用胰岛素泵进行胰岛素持续皮下注射方案治疗。就两组患儿胰岛素总用量、餐后血糖达标的时间、空腹血糖达标的时间和1型糖尿病治疗总有效率、低血糖在治疗过程中出现率进行比较,并对比治疗前后餐后血糖、空腹血糖、糖化血红蛋白指标。结果泵注组1型糖尿病治疗总有效率明显高于皮下注射组,P<0.05。泵注组低血糖在治疗过程中出现率明显低于皮下注射组,P<0.05。泵注组患儿胰岛素总用量低于皮下注射组,餐后血糖达标的时间、空腹血糖达标的时间均明显短于皮下注射组,P<0.05。治疗前两组餐后血糖、空腹血糖、糖化血红蛋白指标比较无显著差异,P>0.05;治疗后泵注组餐后血糖、空腹血糖、糖化血红蛋白指标降低幅度较大,P<0.05。结论儿童1型糖尿病治疗中不同胰岛素注射方案效果存在差异,采用胰岛素泵持续皮下注射胰岛素效果优于多次皮下注射,可减少胰岛素用量,缩短血糖达标时间,加速患儿血糖改善,有效预防低血糖事件的发生,安全有效,值得推广。 Objective To explore the effects of different insulin injection schemes on children with type1diabetes.Methods A total of92diabetic children treated during Feb.2010and Feb.2017were divided into two groups,46cases in either group.Children in the subcutaneous injection group received multiple subcutaneous insulin injection(3-4times),and children in the pump group received continuous subcutaneous injection from an insulin pump.The total insulin dosage,time of postprandial blood glucose control,time of fasting blood glucose control total effective rate,and incidence of hypoglycemia in the treatment process were com pared.Postprandial glucose,fasting glucose,and glycosylated hemoglobin before and after treatment were compared.Results The total effective rate was significantly higher in the pump group,P〈0.05.The incidence rate of hypoglycemia was lower,total dosage of insulin was less,time of the postprandial blood glucose control and time of fasting blood glucose control were shorter in the pump group,all P〈0.05.There were no significant differences in blood glucose,fasting blood glucose and glycosylated hemoglobin between the two groups before treatment,P>0.05.After treatment,the blood glucose,fasting blood glucose and glycosylated hemoglobin index decreased more significantly in the pump group,P〈0.05.Conclusion Insulin pump is more effective than multiple subcutaneous injection.It can reduce the dosage of insulin,shorten the time of blood glucose con trol,improve blood sugar level,and preventhypoglycem ia events.It is safe and effective and worth promoting.
