摘要
目的:观察血液透析联合血液灌注治疗维持性血液透析合并难治性高血压的临床效果。方法:选取2014年2月—2016年3月我院收治的78例维持性血液透析合并难治性高血压患者,随机分为治疗组和对照组,每组39例,对照组患者给予常规血液透析进行治疗,治疗组在此基础上联合血液灌注进行治疗。比较两组患者治疗前后醛固酮、血浆肾素(RA)及血管紧张素Ⅱ(AngⅡ)浓度变化情况,并测定其血压水平。结果:经治疗后,治疗组的各项监测指标水平均较治疗前明显下降(P<0.05);对照组的各项监测指标水平较治疗前无明显差异(P>0.05)。结论:血液透析联合血液灌注能够有效降低维持性血液透析合并难治性高血压患者的醛固酮、血浆肾素(RA)、血管紧张素Ⅱ(AngⅡ)及血压水平,可作为治疗维持性血液透析合并难治性高血压较为理想的治疗方案。
Objective:To observe the clinical effect of hemodialysis combined hemoperfusion in the treatment of maintenance hemodialysis combined refractory hypertension.Methods:78cases of maintenance hemodialysis combined refractory hypertension patients treated in our hospital from February2014to March2016were randomly divided into the treatment group and the control group,with39cases in each group,the control group patients received regular hemodialysis treatment while the treatment group received hemoperfusion on the basis of the regular hemodialysis treatment.The changes of aldosterone,plasma renin(RA)and angiotensinⅡ(AngⅡ)concentration of two groups before and after treatment were compared and the blood pressure levels were measured.Results:After treatment,the level of monitoring indicators of the treatment group decreased significantly than that before treatment(P<0.05);the level of monitoring indicators of the control group had no significant difference with that before the treatment(P>0.05).Conclusion:Hemodialysiscombined hemoperfusion could effectively reduce aldosterone,plasma renin(RA),angiotensinⅡ(AngⅡ)and blood pressure level ofmaintenance hemodialysis combined refractory hypertension patients and it could be used as an ideal treatment in treating maintenance hemodialysis combined refractory hypertension.
