摘要
观察CA15 - 3和CEA联合检测在乳腺癌术后随访中的价值。空腹取静脉血 ,分别采用免疫放射分析法(IRMA)和放射免疫分析法 (RIA)测定CA15 - 3和CEA。结果是 :(1)乳腺癌术后无复发转移 12 7例 ,其中CA15 - 3增高 3例、CEA增高 2例 ,阳性率分别为 2 .4 %、1.6 %。复发转移 18例 ,其中CA15 - 3增高 15例、CEA增高 8例 ,敏感度分别为 83.3%、4 4 .4 % ,特异度分别为 97.6 %、98.4 %。CA15 - 3和CEA两者均异常 ,在无复发转移组为 0 ,复发转移组 6例 ,其敏感度为 33,3% ,特异度为 10 0 %。联检两者之一阳性 16例 ,其敏感度为 88.9% ,特异度为 96 .1% ,经 χ2 检验 ,联检与单独检测CA15 - 3比较无显著性差异 (P >0 .0 5 )。 (2 )复发或转移乳腺癌患者CA15 - 3和CEA水平显著高于无复发或无转移乳腺癌患者 (P <0 .0 1)。CA15 - 3和CEA的检测可作为乳腺癌术后随访的重要指标 ,检测CA15 - 3和 (或 )
To investigate the follow-up value of combining detection of CA15-3 and CEA in the postoperative patients with breast cancer, CA15-3 and CEA were respectively detected by IRMA and RIA with the fasting vein blood. Results are: (1)the levels of CA15-3 in 3 cases and CEA in 2 cases increase in the 127 postoperative breast cancer patients without recurrence and metastasis, the positive rate is 2.4% and 1.6% respectively. In the 18 cases with recurrence and metastasis, 15 patients were measured with higher CA15-3, 8 patients with higher CEA, the sensitivity is 83.3%, 44.4% and the specificity is 97.6%, 98.4% respectively,16 positive patients were detected by either of the methods, the sensitivity is 88.9% and the specificity is 96.1%. Combining detection had no significant difference compared with detecting CA15-3 alone by χ 2 analysis. No case in patients without recurrence and metastasis and 6 cases in patients with metastasis and recurrence were detected for both CA15-3 and CEA concurrent abnormality, its sensitivity is 33.3% and its specificity and is 100%. (2)the level of CA15-3 or CEA is significant higher in the breast cancer patients with metastasis and recurrence than in those without metastasis and recurrence. The detection of CA15-3 and/or CEA can be an important index for the follow-up to the postoperative breast cancer, and is important for predicting the recurrence and metastasis of breast cancer.
