慢性肾脏病患者血载脂蛋白AI的变化

摘要 目的 探讨慢性肾脏病患者血载脂蛋白AⅠ、(ApoAI)及血脂代谢的变化。 方法 慢性肾脏病患者 72例 ,根据放射性同位素 (99mTc -DTPA)方法测定肾小球滤过率 (GFR)分为三组 :GFR <5 0ml/min组 (G1) ;GFR 5 0 -10 0ml/min组 (G2 ) ;GFR >10 0ml/min组 (G3 )。分别测定血肌酐 (Scr)、尿素氮 (Bun)、甘油三脂 (TG)、总胆固醇 (CHOL)、载脂蛋白AI (ApoAI)、载脂蛋白B (ApoB)、高密度脂蛋白 -胆固醇 (HDL -C)。 结果 G1组 (2 8例 )较G2组 (3 4例 )ApoAI明显下降 (1.0 7± 0 .2 9vs 1.2 3± 0 .3 2 g/L ,P <0 .0 5 ) ,较G3组 (10例 )也明显下降 (1.0 7± 0 .2 9vs 1.5 1± 0 .5 1g/L ,P <0 .0 5 )。G2、G3组间无差别。各组间TG、CHOL、ApoB、HDL -C差异无统计学意义。 结论 慢性肾脏病非透析患者GFR <5 0ml/min时可出现ApoAI水平下降 ,但未发现TG、chol、ApoB、HDL -C改变。 Objective To study the change of serum Apo A I and lipids metabolism in patients with chronic renal diseases.Methods 72 patients with chronic renal diseases were allocated to three groups according to GFR determined by 99m Tc-DTPA. Patients were divided into Group 1 with GFR lower than 50 ml/min,Group 2 with GFR between 50~100 ml/min,and in Group 3 with GFR higher than 100 ml/min. TG?CHOL?ApoA I?Apo B and HDL-C were determined in all cases.Results Concentration of serum Apo A I in Group 1 (1.07±0.29 g/L) was lower than Group 2(1.23±0.32 g/L, P <0.05) and Group 3(1.51±0.51 g/L, P <0.05).No difference was found between Group 2 and Group 3.No difference of TG?CHOL and Apo B and HDL-C was found between all three groups.Conclusions Concentration of serum Apo A I in non-dialysis patients with GFR lower than 50 ml/min decreases, but no changes are found in CHOL?TG? Apo B and HDL-C.
