摘要
目的:探讨MMP-9作为食管癌生物标志物的可靠性。方法:计算机检索Web of Knowledge、Pub Med、Cochrane Library、中文科技期刊全文数据库(VIP)、万方数据库及中国知网(CNKI)等数据库,搜集从建库至2017年12月的相关文献,按照纳入与排除标准筛选文献,最终纳入文献14篇,其中食管癌847例,对照516例,应用Stata MP 14和Meta Disc 1.4软件对纳入文献进行诊断性试验meta分析。结果:Meta分析结果显示,MMP-9在食管癌组织中的表达高于癌旁组织。MMP-9诊断食管癌的灵敏度为0.73(95%CI 0.68~0.77),特异度为0.82(95%CI0.73~0.89),阳性似然比为4.1(95%CI 2.6~6.5),阴性似然比为0.33(95%CI 0.27~0.40),诊断优势比为12(95%CI 7～23),SROC曲线下面积为0. 80(95%CI 0. 77～0. 84)。结论:MMP-9在食管癌组织中的表达升高,可作为诊断食管癌的辅助参考指标之一。
Aim： To investigate the reliability of MMP-9 as biomarker of esophageal cancer. Methods： The databases of Web of Knowledge,Pub Med,Cochrane Library,VIP,Wanfang Database and CNKI were searched by computer,and the related documents were collected till December 2017. According to inclusion and exclusion standards of screening literature,14 articles finally were chosen which included 847 cases of esophageal cancer and 516 cases of control. Stata MP 14 and Meta Disc 1. 4 were used in meta analysis of the literature diagnostic accuracy. Results： Meta-analysis showed that the expression of MMP-9 in esophageal cancer tissue was higher than that in paracancerous tissue. The diagnostic accuracy of MMP-9 was 0. 73（95% CI 0. 68-0. 77）,specificity was 0. 82（ 95% CI 0. 73-0. 89）. The positive likelihood ratio was4. 1（95% CI 2. 6-6. 5）,the negative likelihood ratio was 0. 33（95% CI 0. 27-0. 40）,and the diagnostic odds ratio was12（95% CI 7-23）. The area under the curve in SROC plot was 0. 80（95% CI 0. 77-0. 84）. Conclusion： The expression of MMP-9 in esophageal cancer tissue is increased and MMP-9 could be used as one of the assistant reference indexes for the diagnosis of esophageal cancer.
