摘要
目的观察炔雌醇环丙孕酮联合克罗米芬对多囊卵巢综合征不孕症患者血清激素及排卵情况的影响,为多囊卵巢综合征不孕症的治疗提供临床依据。方法选取2016年1月-2017年12月该院收治的120例多囊卵巢综合征不孕症患者为研究对象,按照治疗方法分为对照组与观察组,每组各60例。对照组患者给予克罗米芬单纯治疗,观察组在对照组用药基础上再联合炔雌醇环丙孕酮给药,记录两组治疗前后的主要血清激素指标及排卵情况,并统计临床治疗总有效率。结果观察组治疗后的成熟卵泡数、宫颈黏液评分和诱排日子宫内膜厚度均显著多于或高于对照组(均P<0.05),两组治疗后主要血清激素指标(T、E2、LH、FSH)值均较治疗前显著降低(均P<0.05),观察组的各项数值均显著低于对照组(均P<0.05)。观察组治疗后妊娠率、排卵率均显著高于对照组(均P<0.05),临床总有效率高于对照组(P<0.05)。结论炔雌醇环丙孕酮联合克罗米芬治疗多囊卵巢综合征不孕症疗效确切,可有效降低血清激素水平,改善排卵情况,提高妊娠率。
Objective To observe the effect of ethinylestradiol and cyproterone acetate combined with clomiphene on serum hormones and ovulation of infertile women with polycystic ovary syndrome（ PCOS）,provide a clinical basis for treatment of infertile women with PCOS. Methods From January 2016 to December 2017,120 infertile women with PCOS treated in Jiangyou 903 Hospital were selected and divided into control group and observation group according to treatment method,60 patients in each group. The patients in control group were treated by clomiphene,and the patients in observation group were treated by ethinylestradiol and cyproterone acetate combined with clomiphene. The main serum hormones and ovulation before and after treatment in the two groups were recorded. The total clinical effective rates in the two groups were compared. Results After treatment,the number of mature follicles,cervical mucus score,and endometrial thickness on the day of ovulation induction in observation group were statistically significantly higher than those in control group（ P〈0. 05）. In the two groups,the levels of main serum hormones（ T,E2,LH,FSH） after treatment were statistically significantly lower than those before treatment（ P〈0. 05）. The levels of main serum hormones（ T,E2,LH,FSH） after treatment in observation group were statistically significantly lower than those in control group（ P〈0. 05）. The pregnancy rate and ovulation rate after treatment in observation group were statistically significantly higher than those in control group（ P〈0. 05）. The total clinical effective rate in observation group was statistically significantly higher than that in control group（ P〈0. 05）. Conclusion The curative effect of ethinylestradiol and cyproterone acetate combined with clomiphene in treatment of PCOS is good,the therapy can effectively reduce the levels of serum hormones,improve ovulation and pregnancy rate.
