摘要
[目的]观察加味调脂方联合双歧三联活菌对非酒精性脂肪性肝病(NAFLD)患者肝功能、血脂以及肝脏超声影像的影响,并评价其临床疗效及安全性。[方法]收集2015年6月~2016年12月在我院感染科就诊的105例患者,按照随机数字表法将其分为西药组、中药组和中药联合益生菌组(益生菌组),每组35例。3组患者均给予包括饮食调摄、运动指导与健康宣教在内的基础治疗。在此基础上西药组加用易善复,口服,2粒/次,3次/d;中药组加用加味调脂方口服,150ml/袋,1袋/次,2次/d;益生菌组在中药组的基础上口服培菲康,2粒/次,2次/d,3组患者均治疗8周。分别于治疗前、后检测患者肝功能[谷丙转氨酶(ALT)、谷草转氨酶(AST)、谷氨酰转肽酶(GGT)]和血脂水平[总胆固醇(TC)、三酰甘油(TG)],检查肝脏超声影像学变化,并对比其临床疗效观察不良反应。选择统计软件SPSS19.0对数据进行分析。[结果]中药组和益生菌组治疗后的临床总有效率分别为85.29%、88.24%,显著优于西药组的68.57%(P<0.05);中药组和益生菌组比较并无明显差异(P>0.05)。治疗后,3组患者肝脏彩超分度较治疗前均有明显改善(P<0.05);其中中药组、益生菌组较西药组改善更为明显(P<0.05),中药组和益生菌组组间比较无明显差异(P>0.05)。3组患者治疗后的肝功能、血脂水平均有明显降低(P<0.05);且益生菌组下降程度明显优于其他2组(P<0.05)。[结论]加味调脂方联合双歧三联活菌能明显改善NAFLD患者的肝功能,降低血脂水平,其临床疗效确切,且无明显不良反应。
[Objective]To observe the liver function,blood lipid and liver ultrasound imaging of NAFLD patients which treated by Jiawei Tiaozhi decoction（LD）combined with Bifid Triple Viable Bacteria（BTVB）,and evaluate the clinical efficacy and safety.[Methods]One hundred and five cases of NAFLD were selected from June 2015 to December 2016 in our hospital.They were randomly divided into western medicine group,Chinese medicine group,Chinese medicine combined probiotics group,35 cases in each group.Three groups of patients were given basic treatment,including diet therapy,exercise guidance and health education.Based on that,western medicine group were treated with Essentiale orally,2 tablets,3 times/day,TCM group with LD oral,150 ml/bag,1 bag each time,2 times/day;TCM combined with probi-otics group were treated with LD and Bifico,2 tablets,2 times/day.Three groups were treated for 8 weeks.Before and after treatment,we tested the liver function（ALT,AST,GGT）,blood lipid level（TC,TG）and observed the ultrasonic imaging changes of liver.The clinical effect was compared and the adverse reactions were observed.[Results]After treatment,the total effective rate of TCM group and TCM combined probiotics group were 85.29%and 88.24%,which was significantly better than 68.57% of the western medicine group（P〈0.05）,and there was no significant difference between the two groups（P〉0.05）.After treatment,the TCM group,TCM and probiotics group improvement of liver color Doppler index were more obvious than western medicine group,and there was no significant difference between the two groups（P〉0.05）.After treatment the levels of ALT,AST,GGT,TC and TG were significantly reduced of three groups.The TCM combined with probiotics group was significantly better than the other two groups（P〈0.05）.[Conclusion]LD combined with BTVB can significantly improve liver function and reduce blood lipid level in patients with NAFLD,and its clinical curative effect was exact,and there is no obvious adverse reaction.
