乳腺癌保乳术后螺旋断层放疗中不同体位托架摆位误差分析 被引量:2

摘要 目的:探讨仰卧位和俯卧位乳腺托架在保乳术后放疗中的摆位误差。方法:将20例乳腺癌保乳术后患者分为两组,各10例。A组使用仰卧托架无热塑膜固定摆位,B组使用俯卧托架加热塑膜固定摆位。所有患者在放疗实施前均行兆伏级CT(MVCT)扫描,与计划系统千伏级CT(KVCT)图像进行在线配准。经过300次MVCT扫描,获取患者左右(X轴)、头脚(Y轴)、腹背(Z轴)、横断面旋转(Roll)方向的摆位误差数据,并对其误差值进行统计分析。结果:仰卧托架无热塑膜组在左右(X轴)、头脚(Y轴)、腹背(Z轴)、横断面旋转(Roll)方向分别为(0.06±2.30)、(0.42±2.44)、(3.23±2.95)mm和(0.25±0.62)度,俯卧托架加热塑膜的数据则分别为(-1.54±3.19)、(1.15±7.90)、(3.50±3.46)mm和(-0.43±0.61)度。仰卧托架无热塑膜组在左右方向和横断面旋转的误差值比俯卧托架加热塑膜小,P<0.05差异有统计学意义。在头脚方向和腹背方向上,P>0.05差异没有统计学意义。结论:乳腺癌保乳术后螺旋断层放疗中使用仰卧托架无热塑膜固定体位比俯卧托架加热塑膜能提高摆位重复性,提高摆位精度,在乳腺癌保乳术后放疗中具有一定的优势。 Objective: To explore the set-up uncertainties of breast tray both in the supine position and in the prone position in radiotherapy after breast conserving surgery. Methods: 20 cases of subjects after breast conserving surgery for breast cancer are randomly divided into two groups,10 cases for each group. Subjects of group A were treated with supine bracket without thermoplastics while subjects of group B were treated with prone bracket with thermoplastics. All patients had megavolt CT( MVCT) scans before radiotherapy,and were registered with the online planned system kilovolt CT( KVCT) images. Data of left and right( X-axis),head and foot( Y-axis),abdomen-back( Z-axis),and transverse section rotation( Roll) were obtained by 300 MVCT scans. The margins of error were analyzed statistically. Results: Group A's data of left and right( X-axis),head and foot( Y-axis),abdomen( Z-axis),and transverse section rotation( roll) were( 0. 06 ± 2. 30),( 0. 42 ± 2. 44),( 3. 23 ± 2. 95) mm,and( 0. 25 ± 0. 62) degree respectively; Group B' s data were(-1. 54 ± 3. 19),( 1. 15 ± 7. 90),( 3. 50 ± 3. 46) mm,and(-0. 43 ± 0. 61) degree respectively. Conclusion: Supine bracket without thermoplastics improves set-up repeatability rate and accuracy rate than prone bracket with thermoplastics in radiotherapy after breast conserving surgery for breast cancer. The result indicates that this position fixation technology,that is,supine bracket without thermoplastics,offers some application prospects for the radiotherapy after breast.
