摘要
目的:分析不同因素对我院肺癌患者药品费用的影响,促进临床合理用药。方法:通过内蒙古自治区人民医院信息系统调取住院肺癌患者医嘱,以医保关系(支付方式)作为解释变量,其他因素作为控制变量进行研究,在初步进行相关性检验和共线性诊断后,最终纳入了16个自变量,从对肺癌患者药品费用的影响角度进行多元回归分析。结果:共收集符合分析条件的医嘱6 237条。使用抗菌药会增加甲类及乙类药品费用,说明使用抗菌药会对药品费用产生正向影响(P<0.01)。医保关系(支付方式)会对药品费用产生影响(P<0.05),自费患者使用更多自费药品,使药品费用上升。使用原研药品会对药品费用产生正向影响(P<0.01)。结论:不同因素会对药品费用产生不同影响,如医保关系、抗菌药、原研药品使用都对药品费用产生影响,今后制定政策应考虑以上因素,促进合理用药。如针对医保患者应加强用药审核,对抗菌药使用应加强管理,严格按照《抗菌药物临床应用指导原则》使用。
Objective： To analyze effect of different factors on drug cost in patients with lung cancer in our hospital,to improve rational drug use. Methods： A population-based retrospective cross-sectional study was carried out on drug expenditures of patients with lung cancer from Inner Mongolian People＇s Hospital. Multiple regression analysis was conducted on the influence of 16 independent variables on the drug cost of lung cancer patients. Results： The use of antibiotics and original medicine had increase the cost of medicine（ P〈0. 01）. Medical insurance had a positive impact on the expenses of various drugs regarding with lung cancer（ P〈0. 05）,which indicated an obviously positive effect of induced demand on this kind of diseases. Conclusion： Different patterns of drug use and expenditure caused by different insurance types imply that the insurance type does impact physicians＇ prescription behaviors to some extent. The positive effects of insurance schemes on expenditure and appropriate use of medicines may rest on the long-term effects of promoting rational use of drugs.
