甲亢患者剪切波速度与促甲状腺激素的相关性及应用价值 被引量:5

Correlation between shear wave velocity of thyroid tissue and thyroid-stimulating hormone in patients with primary hyperthyroidism and its clinical significance

摘要 目的探讨原发性甲亢患者剪切波速度与促甲状腺激素的相关性及应用价值。方法选取43例确诊为原发性甲亢并同意参与研究的患者及45名健康体检者分别定义为甲亢组和正常组,应用声辐射力脉冲成像(ARFI)测量所有研究对象的甲状腺组织的剪切波速度(SWV),并测定受试者血清促甲状腺激素(TSH),采用重复测量方差分析随访数据,进而分析SWV与TSH的相关性,最后用ROC曲线分析SWV的诊断效能。结果甲亢组的SWV和血清TSH均明显低于正常组,差异有统计学意义(P<0.05);相关性分析显示SWV与血清TSH呈正相关(r=0.585,P<0.05);ROC曲线显示SWV有一定的诊断效能,其曲线下面积为0.948。结论原发性甲亢患者的甲状腺组织弹性与血清促甲状腺激素呈正相关,且SWV具有一定的诊断效能,可以考虑对确诊为甲亢的患者在治疗随访期间评估采用超声弹性成像这一方法。 Objective To explore the correlation between shear wave velocity( SWV) of thyroid tissue and thyroid stimulating hormone( TSH) in patients with primary hyperthyroidism and its clinical application value. Methods Forty-three patients who were diagnosed as primary hyperthyroidism( PTH group) in our hospital and 45 healthy subjects( control group)were enrolled in the study. The SWV of thyroid tissue in all the subjects was measured by acoustic radiation pulse imaging( ARFI). The serum levels of TSH were detected,and the correlation between SWV and TSH was analyzed by repeated measurement analysis of variance( ANOVA). The ROC curve was used to analyze the diagnostic efficacy of SWV. Results The SWV and serum levels of TSH in PHT group were( 1. 72 ± 0. 08) m/s and( 1. 26 ± 0. 36) m U/L,respectively,which were significantly lower than those [( 2. 22 ± 0. 08) m/s and( 2. 38 ± 0. 47) m U/L] in control group( P 〈0. 05). The correlation analysis showed that SWV was positively correlated to TSH( r = 0. 585,P 0. 05). Moreover ROC curve showed that SWV had diagnostic efficacy at a certain extent,with its AUC being 0. 948. Conclusion The thyroid tissue elasticity is positively correlated to TSH in patients with primary hyperthyroidism,moreover,SWV has a certain diagnostic efficacy,therefor,ultrasound elasticity imaging can be regarded as an assessment method during treatment and follow-up of patients with primary hyperthyroidism.
