摘要
目的探讨不同透析方式对维持性血液透析患者骨矿物代谢紊乱的影响。方法方便选取2016年1—12月该院收治的108例血液透析患者为研究对象,依透析方式不同分为实验1组(HD+HDF,27例)、实验2组(HD+HP,27例),实验3组(高通量,27例)、对照组(HD,27例)。对比观察4组透析后血钙(Ca)、血磷(P)、全段甲状旁腺素(iPTH)达标率。结果实验1组Ca、P、iPTH达标率分别为51.85%、55.56%、51.85%,实验2组Ca、P、iPTH达标率分别为55.56%、51.85%、48.15%、实验3组Ca、P、iPTH达标率分别为48.15%、51.85%、55.56%,各项指标均优于对照组(P<0.05)。结论 HD+HDF、HD+HP或高通量血液透析能改善部分骨矿物质代谢指标,对促进钙磷平衡、改善骨代谢异常具有积极作用,具备临床推广价值,临床可根据患者实际血透需求合理应用。
Objective To study the effect of different dialysis methods on the bone mineral metabolic disorder of patients with maintenance hemodialysis. Methods Convenient selection 108 cases of hemodialysis patients admitted and treated in our hospital from January to December 2016 were selected and divided into four groups, including test 1 group(HD+HDF,27 cases), test 2 group(HD+HP, 27 cases), test 3 group(high liquid, 27 cases) and the control group,(HD, 27 cases), and the standard-reaching rates of Ca, P, Ipth of the four groups after dialysis were compared and observed. Results The standard-reaching rates of Ca, P, iPTH in the test 1 group, in the test 2 group and test 3 group were respectively 51.85%,55.56%, 51.85%, 55.56%, 51.85%, 48.15% and 48.15%, 51.85%, 55.56%, various indexes were better than those in the control group(P〈0.05). Conclusion HD+HDF, HD+HP or high-flux hemodialysis can improve some bone mineral metabolic indexes, which has a positive effect on promoting the calcium phosphate balance and improving the bone metabolic abnormality, and it is of clinical promotion value, and we should conduct the rational application according to the practical hemodialysis demands.
