摘要
目的 :观察中西医结合治疗急性下肢深静脉血栓形成的疗效及作用机理。方法 :应用尿激酶和中药制剂静滴 ,配合中药口服及外敷 ,治疗 2个疗程 ,观察治疗前后临床症状、体征、血液流变学、内皮细胞功能的变化。结果 :在治疗的 86例中 ,临床治愈 2 4例 (2 7 91% ) ,显效 4 5例 (5 2 32 % ) ,进步 10例 (11 6 3% ) ,无效 7例 (8 14 % ) ,总有效率 91 86 %。治疗前后临床症状、体征、Doppler超声变化显著 ;血液流变学各项指标较治疗前明显下降 (P <0 0 1) ;治疗前后血浆内皮素水平降低 ,组织型纤溶酶原激活剂活性增加 (P <0 0 1)。结论 :中西医结合治疗急性下肢深静脉血栓形成疗效显著 ,能有效地降低血液粘度 ,改善内皮细胞功能 ,防止血栓复发。
Objective:To observe the effect of integrated therapy of Traditional Chinese and western medicine on acute deep venous thrombosis of the lower extremity and to detect the therapeutic mechanism Methods:The therapy consisted of injection of Urokinase and traditional Chinese medicine injection,and combined with herbal medicine taken orally and used externally.After 2 Courses of treatment,changes of clinical symptoms and signs,hemo rheology and the function of vessel endothelium were observed.Results:Clinical cure was resulted in 24 cases(27.91%),obvionsly effective in 45 cases(52.32%),effective in 10 cases(11.63%)and not effective in 7 cases(8.14%),the total effective rate being 91.86% .The changes of clinical symptoms and signs and Doppler Ultrasound examination were obvious;hemo rheologic criteria decreased obviously( P <0 01);the plasms endothelin level decreased and the activity of t-PA increased( P <0 01).Conclusion:The integrated therapy of traditional Chinese and western medicine is significantly effective for acute deep venous thrombosis of the lower extremity by reducing hemoviscosity,improving the function of vessel endothelium and preventing from recurrence of thrombosis.
