摘要
目的探讨晚期癌患者主要照顾者照顾负荷的现况及其影响因素。方法采用便利取样方法,选取2016年5月—2017年3月在乌鲁木齐市某三级甲等肿瘤医院住院治疗的晚期癌症患者主要家庭照顾者作为研究对象,采用一般资料调查表和癌末照顾负荷量表对其进行问卷调查。采用多元线性回归分析其影响因素。结果纳入220例癌症患者主要照顾者,其照顾负荷总均分为(52.56±10.38)分,多元线性回归分析显示,经济压力、照顾影响收入、家庭人均月收入、患者自理程度、总照顾时间进入回归方程(P<0.05)。结论晚期癌症患者主要照顾者的照顾负荷处于中等水平,针对经济压力大、总照顾时间长、患者自理程度差的照顾者,医护人员耐心、详细解释加重照顾者经济负荷的护理、治疗措施,尽到告知义务和尊重照顾者的选择;根据实际要求,严格执行肿瘤单病种、临床路径,以减轻照顾者的经济负荷。定期与照顾者沟通,了解其存在的困难及身心状况。需要给予有针对性的社会支持,以减轻其照顾负荷,改善患者及照顾者的生存质量。
Objective To investigate the current situation of caregiver burden of primary caregiver of patients with advanced cancer and evaluate its influencing factors. Methods Totally 220 primary caregivers of advanced cancer patients were investigated with general information questionnaire and family caregiver burden scale for family caregivers of the terminal cancer patients in a Class ⅢGrade Ⅰ tumor hospital in Urumqi. Results The total score of caregiver burden of primary caregivers of patients with advanced cancer was 52.56 ±10.38. Multiple linear regression analysis showed that the main influencing factors of the burden of care were economic pressure, impact of care on the income, per capita monthly income of the family, self-care ability of the patients and total care time（P〈0.05）. Conclusion The overall caregiver burden of primary caregivers of patients with advanced cancer remains in medicate level. Regular communication with the caregivers, alleviation of the economic pressure and targeted comprehensive social support are beneficial to reducing caregiver burden and improving the quality of life of patients and caregivers.
