摘要
目的:脑卒中患者的高致残率和慢性病程,给其家庭和照顾者带来了严重的精神心理和体力压力。面对长时间的照顾负担,照顾者特别是主要照顾者往往表现出疲惫,社交活动受限和自我时间被占用,甚至沮丧、悲伤、或忧郁,使他们身心产生巨大改变,以至于生活缺乏满足感,从而影响生活质量。资料来源:应用网络搜索引擎www.google.com,Medline检索国内外,主要是国外1980-01/2001-12间关于脑卒中主要照顾者的相关文献,检索词:脑卒中(stroke),主要照顾者(caregiver),生活质量(lifequality)。资料选择:选择脑卒中主要照顾者,主要照顾者生活质量,生活质量相关因素等相关文献60篇,排除非随机研究原著性文献。资料提炼:在60篇文献中,内容呈不同程度重复的有25篇,给予删除;对35篇文献进行分类整理,用于综述,其中12篇选用为参考文献。资料综合:脑卒中主要照顾者仅仅是指家庭成员而言,其生活质量内涵和外延都非常广泛,可以从主客观两个方面来测评。脑卒中患者在生理、心理、家庭和社会等方面影响着照顾者的生活质量。在脑卒中患者长期康复过程中,主要照顾者存在很高的压力负担,照顾者生活质量的影响因素主要包括:患者、照顾者的本身特性;照顾者的压力感受和社会支持等方面。
OBJECTIVE: The higher disabling rate and chronic course of stroke patients bring serious pressure on psychology and physical strength to the family and caregivers.With the long time burden,caregivers, especially the main caregivers often show tired,limited social activities, no private time,even dejection, sadness, depression, or melancholy,and all of these lead to great changes in their physically and mentally.So that they are not satisfied with the life, and thereby, the life quality is affected.DATA SOURCES:Using the network searching engine“ www.google.com' and “ Medline' with the key terms “ stroke, caregiver, life quality', we searched foreign relevant documents about the caregivers of stroke patients from January 1980 to December 2001.STUDY SELECTION: A total of 60 articles were collected, which were relevant to caregiver of stroke patients, the life quality of caregivers, the related factors of life quality, etc., and the non randomized original documents were excluded.DATA EXTRACTION:In 60 articles, 25 repeated ones in various degrees were excluded; the rest 35 articles were classified, which were used for the summary, and among which 12 were selected as the reference documents.DATA SYNTHESIS: The caregivers of stroke patients just referred to the family members of the patients,whose intension and epitaxy of life quality were very extensive. We could assess them from subjective and objective respects.The stroke patients influenced the life quality of the caregivers in such aspects as physiology, psychology, family and society, etc.During the long term rehabilitation course, high pressure burden was indicated in caregivers.The influence factors of the life quality of the caregivers included the own characters of patients and caregivers, the pressure sensation, the society supporting, etc.CONCLUSION: In the course of rehabilitation of the stroke patients, the caregivers have very high pressure burden, which can decrease the life quality of caregivers, and can influence the rehabilitation the patients indirectly at the same time. However, while influenced by the stroke patients and the caring works, if the caregivers can get the understanding and supporting from other people or society, they can reduce the pressure in order to improve their life quality, finally, the caring quality can be improved obviously.
