摘要
目的研究慢性心力衰竭(CHF)患者血清肿瘤抗原125(CA125)、肿瘤坏死因子α(TNF-α)以及高敏C反应蛋白(hsCRP)水平在治疗前后的变化及其与心功能的关系。方法选择2015年6月至2016年12月本院心血管内科收治的CHF患者84例作为CHF组,72例同期健康体检者作为对照组。对照组和CHF组(治疗前、治疗后、随访3个月)通过直接化学发光试剂盒检测CA125水平,应用酶联免疫吸附法(ELISA)测定患者血清TNF-α和hs-CRP水平。心脏超声检测2组受试对象的心功能指数。结果与对照组比较,CHF组治疗前CA125、TNF-α及hs-CRP水平均明显升高,而左心室射血分数(LVEF)显著降低(均P<0.05)。与治疗前比较,CHF组治疗后CA125、TNF-α及hs-CRP水平明显降低,LVEF升高(均P<0.05)。相关性分析显示,CHF组患者血清CA125水平与TNF-α和hs-CRP呈正相关(r值分别为0.597和0.516,P均<0.05),与LVEF呈负相关(r=-0.567,P<0.05)。结论 CHF患者血清CA125、TNF-α和hs-CRP水平升高;CA125水平与LVEF负相关,可作为CHF病情及疗效评估的潜在指标。
Objective To detect changes in serum cancer antigen 125 （CA125）, tumor necrosis factor-alpha （TNF-α）, and high-sensitivity C- reactive protein （hs-CRP） in patients with chronic heart failure （CHF） , before and after treatment, and to examine the relationship between CA125 and cardiac function. Methods From June 2015 to December 2016,84 patients with CHF in our hospital were selected as the observation group and 72 healthy persons were selected as the control group. Fasting venous blood CA125, TNF-α, and hs-CRP levels were measured before and after treatment. The cardiac left ventricular ejection fraction （LVEF） was measured in both groups using echocardiography. Results The levels of CA125, TNF-α, and hs-CRP in the control group were significantly lower than those in the observation group. The LVEF in the control group was significantly higher than in the observation group （P 〈 0.05 ）. After heart failure treatment, the levels of CA125, TNF-α, and hs-CRP significantly decreased in the observation group, and LVEF showed marked improvement （P 〈 0.05 ）. Analysis showed that the serum CA125 level in patients with CHF was positively correlated with TNF-α and hs-CRP levels, and was negatively correlated with LVEF. Conclusion The levels of CA125, TNF-α, and hs-CRP in patients with CHF could have potential for the evaluation of CHF status and treatment results.
