摘要
目的探讨围产期孕妇泌尿生殖道无乳链球菌的感染情况与耐药趋势,为临床合理用药提供参考依据。方法回顾性分析2012年1月-2016年9月围产期孕妇送检的各类泌尿生殖道标本1 162份,采用纸片扩散法(K-B法)进行药敏试验,对红霉素耐药而对克林霉素敏感或中介的菌株做D试验检测,采用WHONET 5.6软件分析药敏试验结果。结果 1 162份标本中分离出无乳链球菌229株,感染率为19.7%;不同标本类型间阴道分泌物检出率最高,占22.0%;不同年龄段41岁~50岁患者感染率最高,占28.4%;无乳链球菌对青霉素G、氨苄西林、头孢曲松耐药率较低,均<10.0%;5年间耐药率总体呈逐年上升趋势,D试验阳性率为29.5%。结论临床应重视对围产期孕妇无乳链球菌的筛查,加强耐药性监测,合理用药,治疗无乳链球菌感染首选青霉素G和氨苄西林。
OBJECTIVE To investigate the infection situation and drug resistance oI group B Streptococcus m preg- nant women with urogenital tract infection, so as to provide a basis for clinical rational use of drugs. METHODS All kinds of urinary tract specimens from 1162 students of pregnant women from Jan. 2012 to Sep. 2016 were ret- rospectively analyzed. The drug sensitivity test was performed by disk diffusion method (K-B method) ,the eryth- romycin-resistant and ctindamycin-sensitive or intermediate strains were carried out D test,and the results of drug sensitivity test were analyzed by WHONET 5.6 software. RESULTS Totally 229 strains of group B Streptococcus were isolated from 1162 samples,and the infection rate was 19.7%. The detection rate of vaginal secretign was the highest in different types of samples,accounting for 22.0% ,and the infection rate was the highest among 41 years old to 50 years old in different age groups, accounting for 28.40%. Group B Streptococcus had low resistant rates to penicillin G, ampicillin, and ceftriaxone,which were lower than 10.0%. The clrug resistant rate was increasing year by year in the 5 years. The positive rate of D test was 29.5 %. CONCLUSION It should pay attention to the screening of group B Streptococcus in pregnant women in clinic,strengthen the monitoring of drug resistance, use the drugs rationally,and firstly choose penicillin G and ampicillin for the treatment of group B Streptococcus infec- tion.
