摘要
目的对比结肠肛管吻合与超低位结直肠吻合用于低位直肠癌患者术后的近远期肛门功能情况。方法选取2014年6月至2014年12月本院收治的79例低位直肠癌患者为研究对象,随机分为观察组(行直肠癌全切+结肠肛管吻合术,n=38)及对照组(行直肠癌全切+超低位结直肠吻合术,n=41)。比较两组术前、术后3个月及术后12个月的肛门功能。结果术前两组ARP、MSP比较,差异无统计学意义(P>0.05)。术后两组ARP、MSP较术前均有不同程度降低,术后3月、12月对照组的ARP及MSP均明显低于观察组,差异均有统计学意义(P﹤0.05)。观察组术后12月ARP及MSP与术前比较,差异均无统计学意义(P>0.05);对照组术后12月ARP及MSP与术前比较,差异均有统计学意义(P<0.05)。主观评价两组患者肛门功能,观察组术后3个月及术后12个月优良率分别达47.37%、100.00%,均明显优于对照组,差异具有统计学意义(P﹤0.05)。结论与超低位结直肠吻合术相比,结肠肛管吻合术能更好地保留低位直肠癌患者术后的近远期肛门功能。
Objective To investigate the short-term and long-term effect of colon-anal anastomosis with ultralow colorectal anastomosis on anal function in patients with lower colorectal cancer.Methods 79 patients with lower colorectal cancer were recruited from June 2014 to December 2014 and were assigned to treatment group（n=38） and control group（n=41）.Patients in the treatment group received total resection of colorectal cancer and colon-anal anastomosis and patients in the control group received total resection of colorectal cancer and ultralow colorectal anastomosis.Short-term and long-term anal function was compared between the two groups.Results Before the surgery,there was no significant difference in the level of MSP and ARP between the two groups（P〉0.05）.After the operation,ARP and MSP of the two groups decreased from the preoperative level.Levels of ARP and MSP at 3 months and 12 months were significantly lower in the control than in the treatment group（P〈0.05）.Compared with pre-operation,ARP and MSP at12 months did not change in the treatment group（P〉0.05）,but they changed significantly in the control group（P〈0.05）.In the treatment group,patients＇ subjective evaluation on anal function as good was 47.37% at 3 months,and 100.00% at 12 months,which were significantly better than the control group（P〈0.05）.Conclusion Compared ultralow colorectal anastomosis,colon-anal anastomosis can better improve the short-term and long-term anal function in patients with lower colorectal cancer.
