摘要 目的心脏标志物检测用于早期急性心肌梗死患者诊断中的临床价值。方法 50例急性心肌梗死患者作为A组,50例健康体检者作为B组,两组均接受心脏标志物检测,分析其结果。结果 A组心脏标志物检测结果为:肌酸激酶同工酶(CK-MB)为(23.70±1.80)ng/ml,心肌肌钙蛋白I(cTnI)为(39.30±1.80)ng/ml,N末端脑钠肽前体(NT-pro BNP)为(575.0±12.0)pg/ml,D-二聚体(D-D)为(28.60±1.60)μg/ml;B组CK-MB、cTnI、NT-proBNP、D-D分别为(2.01±0.20)ng/ml、(0.24±0.02)ng/ml、(35.0±1.5)pg/ml、(0.25±0.01)μg/ml;B组CK-MB、cTnI、NT-proBNP、D-D值均明显低于A组,差异有统计学意义(P<0.05)。A组单一检测CK-MB、cTnI、NT-proBNP、D-D的阳性率分别为84.0%、76.0%、70.0%、66.0%,联合检测的阳性率为94.0%,心脏标志物联合检测急性心肌梗死患者的阳性率高于单项检测,差异有统计学意义(P<0.05)。结论通过检测心脏标志物的指标水平值,可对急性心肌梗死患者的早期诊断提供参考价值,而各项心脏标志物指标联合检测,诊断效果更佳。 Objective To discuss the clinical value of cardiac markers detection for diagnosis in early acute myocardial infarction patients. Methods There were 50 acute myocardial infarction patients as group A, and 50 health examination people as group B. Both groups received cardiac markers detection to analyze their outcomes. Results Detection outcome of cardiac markers in group A: creatine kinase isoenzyme(CK-MB) as(23.70±1.80) ng/ml, troponin I(cTnI) as(39.30±1.80) ng/ml, N-terminal pro-brain natriuretic peptide(NTproBNP) as(575.0±12.0) pg/ml, D-Dimer(D-D) as(28.6±1.6) μg/ml. Group B had CK-MB, cTnI, NT-proBNP and D-D respectively as(2.01±0.20) ng/ml,(0.24±0.02) ng/ml,(35.0±1.5) pg/ml and(0.25±0.01) μg/ml. Group B had lower CK-MB, cTnI, NT-proBNP and D-D value than group A, and their differences had statistical significance(P〈0.05). Group A had positive rate in CK-MB, cTnI, NT-proBNP and D-D by single detection respectively as 84.0%, 76.0%, 70.0% and 66.0%, and 94.0% by combined detection. Combined detection in cardiac markers had higher positive rate than that in single detection for acute myocardial infarction patients, and the difference had statistical significance(P〈0.05). Conclusion Detection of cardiac markers level provides reference value for early diagnosis of acute myocardial infarction in patients, and combined detection of all cardiac markers shows even better diagnostic effect.
