海南省新诊断2型糖尿病人群血清25羟维生素D水平与各代谢指标的相关性研究 被引量:8

Correlations between 25-hydroxy-vitamin D and metabolic parameters in newly diagnosed type 2 diabetes in Hainan province

摘要 目的观察海南省新诊断T2DM患者的25羟维生素D[25(OH)D]水平,并探讨25(OH)D与各代谢指标的相关性。方法收集海南省102例新诊断T2DM患者及100名NGT人群的资料。测定FPG、TC、TG、LDL-C、HDL-C、钙(Ca)、磷(P)及血清25(OH)D水平。结果 T2DM组BMI、WC、WHR、SBP、DBP、FPG、TC、TG、LDL-C、Ca高于NGT组(P<0.05),HDL-C及25(OH)D低于NGT组(P<0.05或P<0.01)。相关分析显示,所有受试者25(OH)D水平与HDL-C呈正相关(r=0.149,P<0.05),与WC、WHR、FPG、TC、TG、Ca呈负相关(r=-0.170、-0.201、-0.257、-0.266、-0.431、0.211,P<0.05)。T2DM组25(OH)D水平与TC、TG呈负相关(r=-0.273、-0.483,P<0.05)。NGT组25(OH)D水平与TG呈负相关(r=-0.401,P<0.05)。Logistic回归分析显示,低VitD水平(LD)组患糖尿病风险高于高VitD(HD)组(OR:6.983,P<0.05);中VitD水平(MD)组腹型肥胖风险高于HD水平组(OR:2.271,P<0.05);LD及MD水平组TG增高风险高于HD水平组(OR:5.410、2.964,P<0.05);LD水平组HDL-C降低及血压升高的风险高于HD水平组(OR:3.441、5.690,P<0.05)。结论海南省新诊断T2DM患者VitD水平较NGT人群低;VitD缺乏可能是T2DM及MS发病的危险因素。 Objective To investigate the association between 25-hydroxy-vitamin D[25(OH)D]and the metabolic indices in newly diagnosed type 2 diabetes(T2DM) in Hainan province.Methods Clinical data were collected from 102 newly diagnosed T2 DM and 100 normal glucose tolerance controls in Hainan province.Fasting plasma glucose(FPG),total cholesterol(TO,triglyceride(TG).low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),serum calcium(Ca),serum phosphorus(P) and serum 25(OH)D were detected.Results(1) Body mass index(BMI),waist circumference(WC),waist-to-hip ratio(WHR),systolic blood pressure(SBP),diastolic blood pressure(DBP).FPG.TC,TG,LDL-C and Ca were higher in diabetic subjects than controls(P〈0.05);while HDL-C is lower(P〈0.05).(2) Serum 25(OH)D levels in diabetic subjects were significantly lower than controls[(25.30±7.278) vs(28.72 ± 7.196) ng/ml,P〈0.01].(3) The 25(OH)D levels were positively correlated with HDL-C(r=0.149,P〈0.05),and negatively correlated with WC,WHR,FPG,TC,TGand Ca^(2+)(r=-0.170,-0.201,-0.257,-0.266,-0.431,-0.211,respectively,P〈0.05) in all subjects.The 25(OH)D levels and TC,TG showed a negative correlation(r=- 0.273,- 0.483,P〈0.05) in diabetic subjects.The 25(OH)D levels was negatively correlated with TG(r=- 0.401,P〈0.05) in controls.(4) Logistic regression analysis indicated that low vitamin D group(LD) had increased risk of diabetes compared with high vitamin D group(HD)(QR:6.983,P〈0.05);the risk of abdominal obesity in mid vitamin D group(MD) was higher than HD(OP:2.271,P〈0.05);the risk of elevated TG in LD and MD was higher than HD(QR:5.410,2.964,P〈0.05);the risk of elevated blood pressure and decreased HDL-C in LD was higher than HD(OR:3.441,5.690,P〈0.05).Conclusion Vitamin D levels in newly diagnosed T2 DM are lower than those of normal glucose tolerance controls in Hainan province;Vitamin D deficiency may be the risk factor of diabetes and metabolic syndrome.
