摘要
目的:探讨新诊断2型糖尿病(T2DM)患者血清25羟维生素D [25(OH)D]水平与糖脂代谢的相关性。方法:选取2020年1—10月新乡医学院第一附属医院收治的330例新诊断2型糖尿病患者作为T2DM组,另选取同期于新乡医学院第一附属医院进行健康体检的149名人群作为正常对照(NC)组。测定身高、体重、收缩压(SBP)、舒张压(DBP),计算体质量指数(BMI),检测空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血钙(Ca)、血磷(P)及25(OH)D水平。结果:T2DM组BMI、FPG、TG均高于NC组,差异有统计学意义(t=7.48、5.95、4.73,P<0.05),T2DM组血清25(OH)D水平低于NC组,差异有统计学意义(t=3.53,P<0.05),两组患者SBP、DBP、TC、LDL-C、HDL-C、Ca、P水平比较,差异无统计学意义(t=1.89、1.22、1.78、1.55、1.35、1.24、1.05,P>0.05)。相关性分析显示,新诊断2型糖尿病患者血清25(OH)D水平与BMI、FPG、TG呈负相关(r=-0.293、-0.324、-0.235,P<0.05),与其他指标无相关性。以是否患T2DM为因变量,以年龄、性别、BMI、SBP、DBP、TG、TC、LDL-C、HDL-C、Ca、P、25(OH)D等为自变量进行分析,结果显示,TG是2型糖尿病的危险因素,而25(OH)D是其保护因素。结论:2型糖尿病发病与多种因素相关,血清25(OH)D水平低下可能增加T2DM患病风险,对于糖尿病高危人群、糖尿病前期及糖尿病患者,需关注其25(OH)D水平,及时补充维生素D,有利于T2DM的预防和早期治疗。
Objective:To investigate the correlation between serum 25 hydroxyvitamin D[25(OH)D]levels and glucolipid metabolism in patients with newly diagnosed type 2 diabetes mellitus(T2DM).Methods:330 patients with newly diagnosed type 2 diabetes admitted to the hospital from January 2020 to October 2020 were selected as the T2DM group,and another 149 people who underwent health screening in the hospital during the same period were selected as the normal control(NC)group.The height,weight,systolic blood pressure(SBP),diastolic blood pressure(DBP),body mass index(BMI),fasting blood glucose(FPG),triglycerides(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),blood calcium(Ca),blood phosphorus(P)and 25(OH)D levels were measured.Results:BMI,FPG,and TG were higher in the T2DM group than in the NC group,with statistically significant differences(t=7.48,5.95,4.73,P<0.05).Serum 25(OH)D levels were lower in the T2DM group than in the NC group,and the difference was statistically significant(t=3.53,P<0.05).The differences in SBP,DBP,TC,LDL-C,HDL-C,Ca,and P levels between the two groups were not statistically significant(t=1.89,1.22,1.78,1.55,1.35,1.24,1.05,P>0.05).Correlation analysis showed that serum 25(OH)D levels in patients with newly diagnosed type 2 diabetes were negatively correlated with BMI,FPG,and TG(r=-0.293,-0.324,-0.235,P<0.05)and did not correlate with other indicators.The analysis was performed with the presence of T2DM as the dependent variable and age,gender,BMI,SBP,DBP,TG,TC,LDL-C,HDL-C,Ca,P,and 25(OH)D as the independent variables,which showed that TG was a risk factor for type 2 diabetes and 25(OH)D was its protective factor.Conclusion:The development of type 2 diabetes is associated with multiple factors,and low serum 25(OH)D levels may increase the risk of T2DM.For people at high risk of diabetes,prediabetes and diabetes,their 25(OH)D levels need to be paid attention to,and timely vitamin D supplementation is beneficial to the prevention and early treatment of T2DM.
