摘要
目的探讨急性脑梗死(ACI)患者血栓弹力图(TEG)指标与血浆D-二聚体(D-D)、血小板聚集率(PAgT)之间的相关性。方法选取2015年1月~2016年1月期间收集的120例ACI患者(ACI组)与60例健康对象(健康组),检测两组TEG参数(用血栓弹力仪测定),包括凝血反应时间(R)、血细胞凝集成块时间(K)、血细胞凝集成块速率(α角)、凝血综合指数(CI)、凝血块的最大强度(MA);检测两组凝血指标,包括凝血酶原时间(PT)、纤维蛋白原(FIB,用全自动凝血分析仪检测)、血浆D-D(胶体金法检测)、PAgT(用血液聚集仪检测)。结果 ACI组的R(5.36±0.95 min),K(2.34±0.98min),PT(11.88±1.67 s)低于健康组(6.20±1.28 min,2.87±0.87 min,12.40±1.82 s),(t=4.962,3.547,1.911,P均<0.05);ACI组的α角(61.65°±7.84°),CI(-0.21±1.11),MA(58.94±6.14 mm),D-D(0.34±0.08 mg/L),PAgT(66.9%±6.8%),FIB(4.41±0.96 g/L)均高于健康组[α角(56.02±6.94)°,CI(-1.50±1.30),MA(53.86±7.85 mm),D-D(0.24±0.06 mg/L),PAgT(55.4±7.0)%,FIB(2.87±0.88 g/L)],(t=4.714,6.209,5.583,8.550,10.592,7.039,P均<0.05);ACI的D-D,PAgt与TEG参数R,K之间呈显著负相关(r=-0.481,-0.470,-0.504,-0.488,P均<0.05);ACI的D-D,PAgt与TEG参数α角,CI,MA值之间呈显著正相关(r=0.338,0.395,0.427,0.391,0.436,-0.482,P均<0.05)。结论 TEG各参数可在一定程度上反映ACI患者的病情程度,且与患者血浆D-D,PAgt之间有明显相关性。
Objective To investigate the correlation between thrombelastogram （TEG） parameters and plasma D-dimer （D-D） and platelet aggregation rate （PAgT） in patients with acute cerebral infarction （ACI）. Methods From January 2015 to Jan- uary 2016,120 patients with ACI （ACI group）,and 60 healthy subjects （healthy group） were enrolled in the study. The TEG parameters were detected by thromboelastography,including blood coagulation time （R）,blood clotting time （K）,rate of blood clotting （α angle）, coagulation comprehensive index （CI） and the maximum intensity of blood clotting （MA）. Blood coagulation indexes also were detected,including prothrombin time （PT）, fibrinogen （FIB, detected by automatic coagulation analyzer） ,plasma D-D （detected by colloidal gold method） and PAgT （detected by blood aggregation tester）. Results R （5.36±0.95 min）,K （2.34±0.98 rain） and PT （11.88±1.67 s） in the ACI group were shorter than those in the healthy group （6.20±1.28 min,2.87±0.87 min,12.40±1.82 s）, （t= 4. 962,3. 547,1. 911, all P〈0.05）. The α angle （61.65 ±7.84） degrees,CI （-0.21± 1. 11）,MA （58.94±6.14 mm）,D-D （0.34±0.08mg/L）, PAgT （66.9±6.8）% and FIB （4.41±0.96 g/L） in the ACI group were higher than those in the healthy group [（56.02± 6.94） degrees, （-1.50±1.30）, （53.86 ± 7.85 mm）, （0.24 ± 0.06 mg/L）, （55.4± 7 ） %, （2.87±0.88 g/L）], （t = 4. 714,6. 209,5. 583,8. 550, 10. 592,7. 039,all P〈0.05）. In ACI group,D-D,PAgt and the TEG parameters （α angle,CI,MA） were significantly nega- tively correlated （r=-0. 481 ,-0. 470,-0. 504,-0. 488,all P〈0.05）. D-D,PAgt and the TEG parameters （α angle,CI, MA） were significantly positively correlated （r=0. 338,0. 395,0. 427,0. 391,0. 436, -0. 482,all P〈0.05）. Conclusion TEG parameters can reflect the severity of ACI to some extent,and they were significantly correlated with plasma D-D and PAgt.
