摘要
目的:观察三黄益肾胶囊联合糖皮质激素治疗Ig A肾病的临床疗效及其对血清血管内皮生长因子(VEGF)水平的影响。方法:选取本院收治的96例Ig A肾病患者为研究对象,根据随机数字表将患者分为观察组及对照组各48例,对照组给予糖皮质激素治疗,观察组在对照组基础上给予三黄益肾胶囊治疗,2组疗程均为6个月。观察2组患者的治疗效果,检测2组患者治疗前后尿素氮(BUN)、血肌酐(血SCr)、24 h尿蛋白(24 h Uab)、内生肌酐清除率(Ccr)及VEGF等指标的变化。结果:观察组总有效率为95.83%,明显高于对照组的79.16%,差异有统计学意义(P<0.05)。2组患者治疗后BUN、SCr、24 h Uab水平明显降低,Ccr水平明显升高;观察组治疗后BUN、血SCr、24 h Uab明显低于对照组,而Ccr高于对照组(P<0.05)。2组患者治疗后血清VEGF水平均显著下降,观察组较对照组明显(P<0.05)。观察组不良反应率为6.25%,对照组为10.41%,差异无统计学意义(P>0.05)。结论:三黄益肾胶囊联合糖皮质激素治疗Ig A肾病可显著改善患者肾功能,安全有效,其作用机制可能与VEGF表达下调有关。
Objective： To observe the curative effect of IgA nephropathy treated with Sanhuang Yishen Capsule combining with corticosteroids and impact on serum VEGF level. Methods： 96 patients with IgA nephropathy were divided into observation group and control group,each one 48 cases,control group was given corticoste-roids and observation group eas given Sanhuang Yishen Capsule on the basis of control group,two groups weretreated for six months.. The curative effect was eveluated,the levels of blood urea nitrogen（ BUN）,serum creatinine（ SCr）,24 h urine protein（ 24 hU ab）,creatinine clearance rate of change（ Ccr） and VEGF in the two groups before and after treatment were determined. Results： The total effective rate of observation group was95. 83%,that was much higher than that of control group,that was 79. 16%,the difference between the two groups was significant（ P〈0. 05）. The levels of BUN,SCr,24 hU ab of the two groups were dereased,while the level of Ccr was raised,the effects of observation group were much more obvious than those in control group（ P〈0. 05）; serum VEGF level was lowered in the two groups after treatment,that in observation group post- treatment was significant lower than that of control group. The incidence of adverse reactions of the two groups was6. 25% and 10. 41% respective（ P〈0. 05）. Conclusion： Sanhuang Yishen Capsule combining with corticosteroids can improve curative effect of IgA nephropathy patients,improve renal function,safe and effective,and its mechanism may be related to down- regulation of VEGF expression.
