儿童腺样体肥大的CT与MRI检查效果对比 被引量:6

摘要 目的对比儿童腺样体肥大(AH)的CT与MRI检查效果。方法 2011年9月~2014年12月选择在安康市中心医院诊断为AH儿童80例作为观察组,同期选择无临床症状及体征的80例健康儿童作为对照组,两组都进行CT与MRI检查,比较检查结果。结果相对于对照组,观察组CT主要表现为单纯鼻咽顶后壁软组织增厚形成,呈不均匀强化,观察组的腺样体厚度与A/N值都明显高于对照组(P〈0.05)。而观察组MRI主要表现为鼻咽腔变窄,鼻咽顶后壁软组织增厚,T1WI呈均匀中等信号,T2WI呈中等稍高信号,观察组的A值明显高于对照组,而PAS值明显低于对照组(P〈0.05),两组N值对比无统计学差异(P〉0.05)。在观察组80例患儿中,CT检出74例,检出率为92.5%;而MRI检出80例,检出率为100.0%,MRI对儿童AH的检出率明显高于CT(P〈0.05)。结论儿童AH的CT与MRI检查都能比较全面显示病变的情况,能够反映腺样体压迫阻塞上气道的全貌,而MRI的检出率更高。 Objective To compare the examination effects of CT on children's adenoidal hypertrophy (AH) with those of MRI. Methods 80 children diagnosed as AH in the hospital between September 2011 and December 2014 were selected as observation group. Other 80 children without clinical symptoms and signs at the same period were selected as control group. Both groups received the examination by CT and MRI, and the results were compared. Reeults Compared with the CT manifestation in the control group, that in the observation group was mainly soft tissue thickening formation of the nasopharyngeal top posterior wall with heterogeneous enhancement. The adenoid thickness and A/N value of the observation group were all significantly higher than those of the control group (P 〈 0.05). The MRI manifestations of the observation group were mainly nasal cavity narrowing, soft tissue thickening of the nasopharyngeal top posterior wall, uniform intermediate signal in T1WI, and moderate and slightly higher signal in T2WI. The A values in the observation group were significantly higher than those in the control group, while the PAS values were significantly lower than those in the latter group (P 〈 0.05). The difference in the N values between the two groups was not significant (P 〉 0.05). Among the 80 children in the observation group, 74 ones were detected by CT with the detection rate of 92.5%, and 80 ones were detected by MRI with the detection rate of 100.0%. The detection rate of AH by MRI was significantly higher than that by CT (P 〈 0.05). Conclusion The CT and the MRI examination for children's AH can comprehensively display the pathological changes and the upper airway obstruction caused by adenoids oppression. However, the detection rate of MRI is higher.
