摘要
目的: 总结腺样体肥大的平片表现并介绍两种实用的测量方法。材料与方法: 136 例中,男88 例,女48 例。打鼾136 例,张口呼吸126 例。正常组132 例。均常规摄吸气期鼻咽侧位平片。采用测A/n 比值及平行曲线法阅片。结果: (1) 正常组:A/n 比值均≤0 .66 ,鼻咽顶部,后壁软组织边缘不凸过(107 例) 或轻微凸入(25 例) 平行曲线;(2) 异常组:鼻咽顶部,后壁软组织增厚,鼻咽腔变窄。A/n 比值均> 0 .70 ,其中128 例> 0 .80 ,鼻咽顶部、后壁软组织边缘均凸过平行曲线。结论: (1) 摄吸气时的鼻咽侧位片是观察腺样体肥大的简单而准确的方法;(2) 测定A/n 比值用以观察腺样体的厚度,A/n 比值≤0 .66 为正常,> 0 .70 为腺样体肥大,> 0 .80 者为腺样体显著肥大;(3) 用划平行曲线法可勾画出腺样体的长径。
Objective: To summarize the manifestations of adenoidal hypertrophy on plain film in children and to introduce two measuring techniques.Materials and Methods:Eighty eight boys and 48 girls were diagnosed with adenoid hypertrophy. Snoring was presented in 136 cases and mouth open breathing in 126 cases. 132 cases were collected as normal control group. Nasopharyngeal lateral film were taken for both study group and control group. The A/n ratio was measured and the parallel winding lines were drawn on all films.Results:(1)The A/n ratio in normal group was ≤0.66. The margin of soft tissues in nasopharyngeal roof and posterior part didn't project over the parallel winding lines (n=107) or projected over the parallel winding lines slightly (n=25). (2)In study group: the soft tissues in nasopharyngeal roof and posterior part were thickened, the nasopharyngeal meatus were narrowed, all their A/n ratios were >0.70, and 128 of them were >0.80, the margin of soft tissues in nasopharyngeal roof and posterior part all projected over the parallel winding lines.Conclusion: (1)Nasopharyngeal lateral film with inspiration is a simple and correct examination for visualizing adenoid hypertrophy. (2)Measuring of A/n ratio is used for visualizing the adenoid thickness, normal A/n ratio is ≤0.66, >0.70 indicates adenoid hypertrophy, and >0.80 indicates marked adenoid hypertrophy. (3)Drawing parallel winding lines can outline the adenoid length.
