CT定位颅脑钻孔双置管治疗外伤后亚急性硬膜外合并硬膜下血肿的疗效观察 被引量:9

摘要 目的探讨CT定位颅脑钻孔双置管治疗外伤后亚急性硬膜外合并硬膜下血肿的疗效。方法回顾性分析我院2014年1月—2015年6月经CT定位颅脑钻孔双置管治疗外伤后亚急性硬膜外合并硬膜下血肿患者68例临床资料,随机分为治疗组34例,采用YL-1型微创颅脑钻孔设备(北京万福特公司)行颅脑钻孔双置管引流术进行治疗;对照组34例,采用YL-1型微创颅脑钻孔设备行颅脑钻孔单置管分2次引流进行治疗。结果对两组患者的治疗效果、术后并发症如术后残余液量、颅内积气、颅内感染情况及住院时间等进行统计学分析,治疗组明显优于对照组,差异有统计学意义;治疗组的总有效率为91.2%,对照组总有效率为81.4%,两组比较差异有统计学意义(P<0.05)。住院时间治疗组平均为(7.0±1.0)d,明显短于对照组的(13±2)d,且并发症发生率为10.0%,明显低于对照组的34.6%,差异均有统计学意义(P<0.05)。结论经颅脑钻孔双置管治疗外伤后亚急性硬膜外合并硬膜下血肿疗效确切,住院周期短,并发症少,值得临床推广。 OBJECTIVE To investigate CT positioning craniocerebral drilling double catheter after trauma,subacute epidural combined treatment curative effect observation of a subdural hematoma.METHODS Retrospective analysis in January 2014 to June 2015 CT positioning subacute after craniocerebral drilling double catheter treatment of traumatic epidural combined a subdural hematoma patients 68 cases,randomly divided into treatment group(34 cases,the YL-type 1 minimally invasive cerebral drilling equipment than ford(Beijing) cerebral drilling double catheter drainage for treatment;control group of 34 cases,controlled way the YL-type 1 minimally invasive cerebral drilling equipment cerebral drilling single catheter points 2 times drainage for treatment.RESULTS Treatment effect of two groups of patients,the postoperative complications such as postoperative residual liquid volume,intracranial pneumatosis,intracranial infection and hospitalization time and so on carried on the statistical analysis,the treatment group was better than control group,the difference was statistically significant.The total effective rate was 92.5%in treatment group,control group total effective rate was 80.3%,compare the difference was statistically significant(P〈0.05).Average length of hospital stay in treatment group was(7.0±1.0) d,which was significantly shorter than the control group(13±2) d,and complication rates were significantly lower than the control group 34.6%,10.0%were statistically significant differences(P〈0.05).CONCLUSION Subacute after craniocerebral drilling double catheter treatment of traumatic epidural combined a subdural hematoma curative effect,short hospitalization period,fewer complications,which was worthy of clinical promotion.
