摘要
目的探讨放射治疗联合吉非替尼单药治疗晚期非小细胞肺癌(NSCLC)的临床疗效及不良反应。方法回顾性分析2010年2月至2014年2月收治的100例晚期NSCLC复发患者。其中,接受吉非替尼单药治疗50例作为对照组,另50例采用三维适形放射治疗联合吉非替尼单药治疗作为研究组,分析2组的治疗疗效及不良反应。结果治疗6个月后,对照组完全缓解13例,部分缓解8例,总有效率为42%;研究组完全缓解30例,部分缓解13例总有效率为86%,2组比较,差异有统计学意义(P<0.05)。主要不良反应为I度皮疹、Ⅱ度皮疹、I度腹泻、放射性食管炎和放射性肺炎等。对照组未出现放射性食管炎及肺炎,研究组出现放射性肺炎患者13例,2组比较,差异有统计学意义(P<0.05);研究组患者I度皮疹、Ⅱ度皮疹和I度腹泻发生率均高于对照组,但差异均无统计学意义。结论三维适形放射治疗联合吉非替尼针对晚期NSCLC复发疗效好于单用吉非替尼治疗,并对肿瘤进行有效的局部控制,降低转移和复发的可能性。
Objective To investigate the clinical efficacy and toxicity of radiotherapy combined with gefitinib in the treatment of non-small cell lung cancer. Methods 100 non-small cell lung cancer patients who were from February 2010 to February 2014 in our hospital were analysis. 50 patients who received gefitinib treatment as the control group,the other 50 cases using three dimensional conformal radiotherapy combined with gefitinib monotherapy as the study group. Curative effect and adverse reactions in the two groups were observed. Results After 6 months treatment,the total effective rate of the treatment group was 21 /50（ 42%）,and the radiotherapy combined with the gefitinib group was 43 /50（ 86%）,and the difference was statistically significant（ P〈0. 05）. The main adverse reactions were I degree rash,second degree rash,I degree diarrhea,radioactive esophagitis and radiation pneumonia,etc. Radioactive esophagitis and pneumonia was not observed in the control group,whereas I degree rash,second degree rash and I degrees diarrhea in the study group were higher than the control group,the difference was not statistically significant. Conclusion Three dimensional conformal radiotherapy combined with gefitinib in the treatment of advanced non-small cell lung cancer recurrence is better than that of single treatment,and it can effectively control tumor and reduce metastasis and recurrence.
