摘要
目的:比较无托槽隐形矫治器与直丝弓矫治器对成人牙周炎伴有错牙合畸形的矫治效果。方法:选择60名牙周炎伴有错牙合畸形患者(ICON<50分),按治疗方法随机分成实验组29例(无托槽隐形矫治器)和对照组31例(直丝弓矫治器)。所有患者均在完成牙周基础治疗后开始矫正治疗。治疗结束后使用American Board of Orthodontics(ABO)评分标准比较两组患者的疗效差异。于矫正前后分别测量每例患者的牙周健康状况(牙龈指数、龈沟出血指数、菌斑指数、牙周袋探诊深度、临床附着丧失),以此比较两种矫正方法对患者牙周健康的影响。结果:实验组和对照组患者ABO得分分别是28.42±3.42和27.79±3.15(P>0.05)。两组患者矫治开始前,各项牙周指数无统计学差异;矫治结束后,对照组各项牙周指数均显著升高(P<0.05),而实验组仅有菌斑指数升高。结论:对轻度牙周炎继发错牙合畸形患者,无托槽隐形矫治器和直丝弓矫治器都能取得良好的矫治效果,前者对患者牙周健康状况影响更小。
Objective： To compare the effects of treatments on patients with malocclusion induced by adult periodon- titis using bracketless invisible appliance and straight wire appliance. Method： 60 patients with malocclusion induced by adult periodontitis （ICON〈50） were selected and randomly arranged to experimental group （29 patients, bracketless invisible appliance） and control group （31 patients, straight wire appliance）. Each patient accepted periodontal non-surgical treatment before orthodontic treatment. The effects of orthodontic treatment were evaluated using ABO criterion. The patients＇ peri- odontal health status were detected before and after orthodontic treatment （GI, SBI, PLI, PD, CAD, and analyzed to compare the influence of two types of orthodontic treatment on periodontal health status. Result： The average ABO scores of patients with bracketless invisible appliance and straight wire appliance were 28.42±3.42 and 27.79±3.15 （P 〉0.05）. There are no differences in periodontal health status between two groups before orthodontic treatment. After orthodontic treatment, the pe- riodontal parameters of control group significantly increased, but only plaque index （PI） in experimental group significantly increased. Conclusion：To patients with mild malocclusion induced by adult periodontitis, there are no differences between the effects of treatment with bracketless invisible appliance and straight wire appliance, but the former has less disadvantages on veriodontal health status.
