摘要
目的:比较不同正畸术对牙周炎正畸治疗患者的疗效和龈沟液及血清p21活化激酶5、白细胞介素-1β(IL-1β)、β-防御素2(HBD-2)水平的影响。方法:选取2007年1月～2011年3月于我院进行正畸治疗的48例牙周炎患者为研究对象,将其随机分为A组和B组,每组各24例。A组采用固定矫治器进行治疗,B组采用钢丝结扎联合复合树脂夹板治疗,统计并比较两组患者的治疗总有效率及治疗前、治疗后1、3个月的牙龈指数(GI)、牙周袋深度(PD)、牙松动度(TM)、龈乳头探诊出血指数(PBI)、上颌骨突度(SNA)、下颌骨突度(SNB)、上、下颌骨间的矢状关系(ANB)、龈沟液及血清p21活化激酶5、IL-1β、HBD-2水平。结果:A组的治疗总有效率高于B组,治疗后1、3个月的GI、PD、TM、PBI均小于B组,SNA、SNB、ANB改善幅度大于B组,龈沟液及血清p21活化激酶5、IL-1β水平低于B组,HBD-2水平高于B组,差异均有统计学意义(均P<0.05)。结论:固定矫治器在牙周炎正畸治疗患者中的综合优势明显,可显著改善患者的多项评估指标。
Objective：To compare the effect and influence of different orthodontics for gingival crevicular fluid and serum p21-activated kinase 5,IL-1β,HBD-2 of patients with periodontitis.Methods：48 patients with periodontitis treated with orthodontic in our hospital from January 2007 to March 2011 were selected as research object,and they were randomly divided into group A and group B with 24 cases in each group.The group A were treated with fixed appliance,and the group B were treated with ligature wire combined with composite resin splint,then the total effective rate,GI,PD,TM,PBI,SNA,SNB,ANB,gingival crevicular fluid and serum p21-activated kinase 5,IL-1β,HBD-2 before and 1 and 3 month after treatment of the two groups were analyzed and compared.Results：The treatment total effective rate of group A was significantly higher than that of group B;GI,PD,TM,PBI of group A 1 and 3 month after treatment were significantly smaller than those of group B;improvement range of SNA,SNB,ANB of group A were all significantly bigger than those of group B;gingival crevicular fluid and serum p21-activated kinase 5,IL-1β were all significantly lower than those of group B;serum HBD-2 of group A was significantly higher than that of group B,and there were all significant differences between the two groups（all P〈0.05）.Conclusion：The comprehensive advantage of fixed appliance for periodontitis treated with orthodontic treatment is obvious,and it can significantly improve various evaluation indexes.
