HPVL1蛋白的表达与HPVDAN亚型关系的研究

摘要 目的探讨子宫颈病变中HPVL1蛋白的阳性表达与HPVDNA感染亚型的关系。方法选取已知HPVDNA感染阳性患者的宫颈细胞学标本145例,采用以免疫组织/细胞化学和非同位素标记核酸分子杂交技术为基础,在免疫水平和核酸水平上对其进行HPVL1蛋白检测。结果 HPVL1蛋白总阳性率为44.14%(64/145),其中HPV混合感染型的HPVL1蛋白的阳性率57.63%(34/59),高于HPV单一感染型34.88%(30/86),两者比较差异有统计学意义(χ2=7.34,P〈0.05)。HPVL1蛋白的阳性表达在不同年龄段之间的无显著差异,在40岁以下的阳性率为33.85%(22/65),41~50岁的阳性率为50.94%(27/53),50岁以上的患者阳性率为55.56%(15/27)两者比较差异无统计学意义(χ2=5.22,P〉0.05)。HPV感染亚型的HPVL1蛋白的阳性率分别为:HPV56型68.75%(11/16),HPV35型66.67%(2/3),HPV33型66.67%(14/21),HPV11型60.00%(3/5),HPV53型52.38%(11/21),HPV43型50.00%(2/4),HPV59型50.00%(1/2),HPV58型44.00%(11/25),HPV6型40.00%(2/5),HPV52型37.14%(13/35),HPV45型33.33%(1/3),HPV16型29.03%(9/31),HPV68型25.00%(2/8),HPV18型20.00%(3/15),HPV31型14.29%(1/7),HPV66型12.50%(1/8),HPV55型0.00%(0/2),HPV44型0.00%(0/1)。在HPV感染亚型中HPV56型HPVL1蛋白阳性率明显高于HPV44型,两者之间差异有统计学意义(χ2=6.75,P〈0.05)。结论 HPVL1蛋白的阳性表达与患者的年龄无关,与HPV感染亚型密切相关,HPVL1蛋白联合HPVDNA亚型的检测对宫颈病变的治疗具有重要指导意义,这二者可以作为宫颈病变患者的常规检测指标,HPVL1蛋白的检测对感染HPVDNA的患者的治疗及预后的评估提供了可靠的依据,具有重要的指导意义及临床应用价值。 Objective To study the relationship between the HPVL1 protein positive expression of cervical lesions and the infection of HPVDAN subtypes. Methods Select known HPVDNA positive patients of cervical cytology specimens of 145 cases of infection,In order to isotope labeling nucleic acid molecular hybridization and immune tissue/cell chemical technology as the foundation,at the same time using nucleic acid and the immune level of double detecting pathogens of infectious diseases of human papilloma virus capsid protein antibody(HPVL1) method on 145 cases of cervical tissue specimens of TCT HPVL1 test. Results The total Positive rate of HPVL1 protein in cervical cells exfoliated lesions was44.14%(64/145). The positive rate of HPV type mixed infection HPVL1 protein 57.63%(34/59) was higher than the single infection of HPV 30/86(34.88%). There were statistical difference between them(χ2=7.34,P0.05). There were not significant difference of positive expression of HPVL1 protein in different age. The positive rate was 40 under the age of 33.85%(22/65),the positive rate of 41~50between the ages of 50.94%(27/53),The positive rate in patients over 50 years old was 55.56%(15/27)(χ2=5.22,P0.05). Subtypes of HPV infection HPVL1 protein positive rate is respectively:HPV56type 68.75%(11/16),HPV35 type 66.67%(2/3),type HPV33 66.67%(14/24),HPV11 type 60.00%(3/5),HPV53 type 52.38%(11/21),HPV43 type 50.00%(2/4),HPV59 type 50.00%(1/2),HPV58 type44.00%(11/25),HPV6 type 40.00%(2/5),HPV52 type 37.14%(13/35),HPV45 type 33.33%(1/3),HPV16 type 29.03%(9/31). HPV68 type 25.00%(2/8),HPV18 type 20.00%(3/15),HPV31 type14.29%(1/7),HPV66 type 12.50%(1/8),HPV55 type 0.00%(0/2)HPV44 type 0.00%(0/1). The positive rate of HPV56 was higher than which of HPV44(χ2=6.75,P0.05). There were significant difference between them. Conclusions The expression of HPVL1 protein was not related to the age of the patient.The positive rate of HPVL1 protein expression was correlated with the subtypes of HPVDNA infection.The detection of the HPVL1 protein and HPVDNA subtypes have important guiding significance for the treatment of cervical lesions,which can be used as Conventional detection indicators of the Patients with cervical lesions,The detection of HPVL1 protein provides a reliable basis,have important guiding significance to the clinical application value for the reatment and prognosis of patients infected with HPVDNA evaluation provides a reliable basis.
