单侧经皮椎体成形术治疗骨质疏松性上胸椎骨折的临床疗效观察 被引量:13

Observation on the clinical efficacy of percutaneous vertebroplasty in the treatment of upper thoracic fractures

摘要 目的:总结和讨论使用经皮椎体成形术(PVP)治疗骨质疏松性上胸椎骨折的临床治疗效果。方法对我院于2008~2014年收治的诊断为骨质疏松性骨折且骨折部位为上胸椎并行PVP手术治疗的51例患者行回顾性分析。比较和总结患者手术前和术后3 d、6个月时的VAS疼痛评分、Cobb角、椎体前缘高度、Mac Nab疗效及并发症等指标。结果所有患者(均至少随访6个月)术后、术后6个月的VAS评分、Cobb角及椎体前缘高度较术前均有明显的改善。Mac Nab疗效评价:优38例,良8例,可3例,差2例,优良率为90.2%。1例患者在手术中出现骨水泥渗漏,但无明显症状;1例患者在术后9个月时发生相邻椎体再次骨折,余无明显并发症出现。结论 PVP能够有效治疗骨质疏松性上胸椎骨折且相对安全,值得在临床上进一步的推广。 ObjectiveTo evaluate the clinical outcomes of percutaneous vertebroplasty for upper thoracic fractures.Methods51 patients who were admitted in our hospital from 2008 to 2014 and diagnosed as upper thoracic fractures were cured by percutaneous vertebroplasty. Clinical outcomes were evaluated by visual analogue scale (VAS), Cobb, the heights of anterior and modified Macnab criteria at reoperation, 3 days after surgery and 6 weeks after surgery.ResultsAl patients were folowed up at least 6 months after the operation and the excelent was 90.2%. The results of postoperative VAS score Cobb and the heights of anterior after surgery were obviously improved compared with preoperative evaluation (P〈0.05). 1 patient had the leakage of bone cement who felt no sick and 1 patient had vertebral fractures around vertebral lesion.ConclusionPercutaneous vertebroplasty could effectively cure upper thoracic fractures which could be further used in clinic.
